TY - JOUR AU - Sakrajai, Parichat AU - Arsanatong, Dechatorn AU - Tiangphak, Nontaphak AU - Wongnijasil, Soopawadee AU - Pichaichanlert, Apiradee AU - Jodking, Prompisit AU - Srisaenpang, Sompong PY - 2013/08/22 Y2 - 2024/03/29 TI - Completeness of the Received Information of Patients before Signing in Preoperative Informed Consent JF - Srinagarind Medical Journal JA - SRIMEDJ VL - 27 IS - 1 SE - Original Articles DO - UR - https://li01.tci-thaijo.org/index.php/SRIMEDJ/article/view/11079 SP - 29-36 AB - <p><strong><span style="text-decoration: underline;">หลักการและวัตถุประสงค์</span></strong><span style="text-decoration: underline;">:</span><strong> </strong> ผู้ป่วย<span style="color: #333333;">มีสิทธิที่จะได้รับทราบข้อมูลอย่างเพียงพอ</span>ก่อนการรักษาโดยการผ่าตัด ยังไม่เคยมีการศึกษาเรื่องนี้มาก่อนในประเทศไทย การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความชุกของผู้ป่วยที่ได้รับแจ้งข้อมูลอย่างครบถ้วนก่อนการลงนามในใบยินยอมผ่าตัด และปัจจัยที่อาจมีความสัมพันธ์กับการได้รับแจ้งข้อมูลอย่างครบถ้วนดังกล่าว</p><p><strong><span style="text-decoration: underline;">วิธีการศึกษา</span></strong><span style="text-decoration: underline;">:</span><strong> </strong>เป็นการวิจัยเชิงพรรณนา ประชากรศึกษาเป็นผู้ป่วยที่นัดมาเข้ารับการผ่าตัดในโรงพยาบาลตติยภูมิแห่งหนึ่งในจังหวัดขอนแก่น เลือกตัวอย่างโดยการสุ่มตามเวลา ได้ตัวอย่าง 80 ราย เก็บข้อมูลโดยใช้แบบสัมภาษณ์   การได้รับแจ้งข้อมูลอย่างครบถ้วนหมายถึงครบทั้ง 6 องค์ประกอบที่ระบุไว้ในใบยินยอมผ่าตัด (วิธีการผ่าตัด ผลดี ผลข้างเคียง ความเสี่ยง ทางเลือกอื่นของการรักษา และความเป็นไปได้ที่อาจต้องทำการตรวจรักษาหรือผ่าตัดเพิ่มเติมอย่างทันทีทันใดในระหว่างกำลังผ่าตัด)</p><p><strong><span style="text-decoration: underline;">ผลการศึกษา</span></strong><span style="text-decoration: underline;">:</span> เก็บข้อมูลได้ร้อยละ 100 เป็นชายร้อยละ 38.8 อายุเฉลี่ย 51.9 ปี  เข้ารับการผ่าตัดในกลุ่มงานศัลยศาสตร์ นรีเวชวิทยา และศัลยกรรมกระดูกและข้อ ร้อยละ 47.5, 26.3 และ 26.3 ตามลำดับ  ความชุกของผู้ป่วยที่ได้รับแจ้งข้อมูลอย่างครบถ้วนเท่ากับร้อยละ 17.5 (95%CI: 10.2%, 28.0%) โดยได้รับแจ้งเรื่องวิธีการผ่าตัด ผลดีของการผ่าตัด ผลข้างเคียงของการผ่าตัด ความเสี่ยงของการผ่าตัด ทางเลือกอื่นของการรักษา และความเป็นไปได้ที่อาจต้องทำการตรวจรักษาหรือผ่าตัดเพิ่มเติมอย่างทันทีทันใดในระหว่างกำลังผ่าตัด ร้อยละ 93.8, 65.0, 60.0, 57.5, 57.5 และ 45.0 ตามลำดับ ปัจจัยที่อาจมีความสัมพันธ์กับความครบถ้วนในการได้รับแจ้งข้อมูล ได้แก่ การที่ผู้ป่วยจำข้อมูลที่แพทย์อธิบายได้ครบถ้วน (p-value = 0.039), กลุ่มงานที่ผู้ป่วยเข้ารับการผ่าตัด (p-value = 0.059) และระยะเวลาของการอธิบายข้อมูล (p-value = 0.062)</p><p><strong><span style="text-decoration: underline;">สรุป</span></strong><span style="text-decoration: underline;">:</span> ผู้ป่วยน้อยกว่า 1 ใน 5 ได้รับแจ้งข้อมูลครบถ้วนก่อนการลงนามในใบยินยอมผ่าตัด ดังนั้นแพทย์ควรตรวจสอบองค์ประกอบในการให้ข้อมูลกับใบยินยอมผ่าตัด และใช้เวลาอธิบายให้เหมาะสมกับสภาวะของผู้ป่วยแต่ละราย</p><p><strong><span style="text-decoration: underline;">คำสำคัญ</span></strong><span style="text-decoration: underline;">:</span> ใบยินยอมผ่าตัด, ความครบถ้วน, ข้อมูล, การลงนาม</p><p> </p><p> </p><p><strong><span style="text-decoration: underline;">Background and Objectives</span></strong><span style="text-decoration: underline;">:</span> Patients have right to receive complete information before any operation. There was no such study in Thailand. This study aimed to determine prevalence and its affecting factors of the patients receiving complete information before signing in preoperative informed consent.</p><p><strong><span style="text-decoration: underline;">Methods</span></strong><span style="text-decoration: underline;">:</span> This descriptive study had study population as operative patients at a tertiary hospital in Khon Kaen province. Random samples of 80 patients were interviewed with oral questionnaire. Completeness of information was defined to cover all six components in the preoperative informed consent (surgical method, benefit, adverse effect, risk, alternative treatment and possibility of emergency-additional-surgical procedure during the operation).</p><p><strong><span style="text-decoration: underline;">Results</span></strong><span style="text-decoration: underline;">:</span> Response rate was 100.0%. The patients were 38.8% male with average age of 51.9 years. They got operation at department of surgery, gynecology and orthopedics for 47.5%, 26.3% and 26.3% respectively. Prevalence of the patients receiving complete information was 17.5% (95%CI: 10.2%, 28.0%). The patients were informed about surgical method, benefit, adverse effect, risk, alternative treatment and possibility of emergency-additional-surgical procedure during the operation for 93.8%, 65.0%, 60.0%, 57.5%, 57.5% and 45.0% respectively. Factors that might associate with the completeness were patient's capability to remember the received information (p-value = 0.039), department of operation (p-value = 0.059) and duration of explaining (p-value = 0.062).</p><p><strong><span style="text-decoration: underline;">Conclusion</span></strong><span style="text-decoration: underline;">:</span> Less than one-fifth of the patients received complete information before signing in preoperative informed consent. So a physician should check components of giving information with the preoperative informed consent.
