Proportion of Long-Term Care Needs among Community dwelling Elderly in Ban Bung Mak Lan Health Promotion Hospital, Kumphawapi District, Udon Thani Province, Thailand

Abstract

Background and objectives: The proportion and number of older people in Thailand are increasing dramatically and long-term care services should be provided in accordance with the context of the community. This study aimed to identify the proportion of long term care needs and related factors of the community dwelling elderly at Bung Mak Lan Health Promoting Hospital, Pako subdistrict, Kumphawapi District, Udon Thani Province.

Methods: A descriptive study was conducted. The study population comprised of the elderly aged 60 years and older. 890 samples were selected through simple random sampling method. The study tool was the questionnaire which divided into three parts: Sociodemographic, Activities of Daily Living (Barthel ADL index: ADL) and Health status. The data were collected by trained interviewers. The gathered data were analyzed by mean, SD, frequency, percentage, Odds Ratio, 95%CI and Adjusted Odds ratio.

Results: Response rate was 85.28%. Most of the elderly  samples were females, married, completed the primary school. This study found the proportion of long term care needs was 2.5% (95%CI: 1.15, 3.81). Factors related to long term care needs were working status (AOR=0.01, 95% CI: 0.001, 0.03) Buddhism prayer group (AOR=0.24, 95%CI: 0.06, 0.93) Perceived health status (AOR= 9.21, 95% CI: 2.26, 37.42) and disability registered (AOR=15.93, 95% CI: 2.75, 92.27).

Conclusions: The elderly should be promoted to work and join Buddhism prayer group. The elderly perceived health status that they need some care and should have health promotion, continuing care and for disability elderly should be developed health service systems in rehabilitation due to improve their quality of life and reduced the long term care needs of the elderly in community.

หลักการและวัตถุประสงค์: สัดส่วนและจำนวนผู้สูงอายุในประเทศไทยเพิ่มขึ้นอย่างมาก และควรจัดให้มีบริการการดูแลระยะยาวให้สอดคล้องกับบริบทของชุมชน การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อหาสัดส่วนความต้องการการดูแลระยะยาวและปัจจัยที่เกี่ยวข้อง ในผู้สูงอายุที่อาศัยอยู่ในพื้นที่รับผิดชอบของโรงพยาบาลส่งเสริมสุขภาพตำบลบ้านบุ่งหมากลาน ตำบลปะโค อำเภอกุมภวาปี จังหวัดอุดรธานี

วิธีการศึกษา: เป็นการศึกษาเชิงพรรณนา ประชากรศึกษาคือ ประชากรที่มีอายุ 60 ปีบริบูรณ์ขึ้นไป คำนวณขนาดตัวอย่างได้ 890 ราย สุ่มตัวอย่างแบบอย่างง่าย เครื่องมือที่ใช้ในการศึกษาคือ แบบสัมภาษณ์ประกอบไปด้วย 3 ส่วนคือ ข้อมูลทั่วไป แบบประเมินความสามารถในเชิงปฏิบัติ ดัชนี บาร์เธลเอดีแอล (Barthel ADL index: ADL) และภาวะสุขภาพ เก็บรวบรวมข้อมูลโดยพนักงานสัมภาษณ์ที่ผ่านการอบรมแล้ว วิเคราะห์ข้อมูลด้วยค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ความถี่ ร้อยละ Odds Ratio 95%CI และ Adjusted Odds ratio

ผลการศึกษา: อัตราตอบกลับคิดเป็นร้อยละ 85.28 ผู้สูงอายุส่วนใหญ่เป็นเพศหญิง สถานภาพสมรสคู่ จบการศึกษาในระดับประถมศึกษา สัดส่วนของความต้องการการดูแลสุขภาพระยะยาวของผู้สูงอายุในชุมชน ร้อยละ 2.5 (95%CI: 1.15, 3.81) และปัจจัยที่เกี่ยวข้องกับความต้องการการดูแลสุขภาพระยะยาวของผู้สูงอายุในชุมชน ได้แก่ การประกอบอาชีพ (AOR 0.01 95% 0.001, 0.03) การเป็นสมาชิกกลุ่มสวดมนต์ (AOR 0.24 95%CI 0.06, 0.93) การรับรู้ภาวะสุขภาพปัจจุบัน (AOR 9.21 95% 2.26, 37.42) และการขึ้นทะเบียนผู้พิการ (AOR 15.93 95% 2.75, 92.27)

สรุป: ควรส่งเสริมให้ผู้สูงอายุทำงาน เข้าร่วมกิจกรรมกลุ่มสวดมนต์ และกลุ่มผู้สูงอายุรับรู้ว่าตนเองมีภาวะสุขภาพที่ต้องการดูแล ควรมีการส่งเสริมสุขภาพ ดูแลสุขภาพต่อเนื่อง รวมถึงพัฒนาระบบการดูแล ผู้สูงอายุที่มีความพิการ เพื่อฟื้นฟูสภาพ เพื่อให้ผู้สูงอายุมีคุณภาพชีวิตที่ดี และลดความต้องการการดูแลสุขภาพระยะยาวของผู้สูงอายุในชุมชน ต่อไป
