Validity of Kyphosis Measure using the Occiput-Wall Distance and Effectiveness of Outcomes on the Identification of Impairments on Functional Endurance

Abstract

หลักการและวัตถุประสงค์ : การตรวจประเมินภาวะกระดูกสันหลังค่อมโดยใช้ระยะจากผนัง(occiput-wall distance) เป็นวิธีที่นิยมใช้ในทางระบาดวิทยา แต่ยังไม่มีรายงานความเที่ยงของผลการวัดเปรียบเทียบกับวิธีมาตรฐาน ดังนั้น การศึกษานี้จึงมีวัตถุประสงค์หลักเพื่อหาความสัมพันธ์ของการประเมินภาวะกระดูกสันหลังค่อมโดยใช้ระยะจากผนังและวิธีมาตรฐานโดยใช้ flexicurve และเปรียบเทียบความแตกต่างของความทนทานในการทำงานในผู้สูงอายุที่มีความรุนแรงของภาวะกระดูกสันหลังค่อมระดับต่างๆที่ประเมินโดยใช้ระยะจากผนัง

วิธีการศึกษา : การศึกษานี้เป็นแบบภาคตัดขวางในผู้ที่มีอายุตั้งแต่ 60 ปีขึ้นไป แบ่งอาสาสมัครออกเป็นจำนวน 3 กลุ่มตามความรุนแรงของภาวะกระดูกสันหลังค่อมที่ประเมินโดยใช้ระยะจากผนัง อาสาสมัครทุกรายได้รับการประเมินภาวะกระดูกสันหลังค่อมโดยใช้ระยะจากผนังและ flexicurve และประเมินความทนทานในการทำงานโดยใช้ระยะทางการเดินในเวลา 6 นาที

ผลการศึกษา : ผลการศึกษาพบว่าการประเมินภาวะกระดูกสันหลังค่อมโดยใช้ระยะจากผนังมีความสัมพันธ์ในระดับดีเยี่ยมกับผลการวัดโดยใช้ flexicurve (r= 0.925, p<0.001) โดยอาสาสมัครทั้ง 3 กลุ่มมีความแตกต่างของระยะทางการเดินในเวลา 6 นาที อย่างมีนัยสำคัญทางสถิติ (p<0.005)

สรุปผล : การวัดภาวะกระดูกสันหลังค่อมโดยใช้ระยะจากผนังมีความเที่ยงและประสิทธิภาพในการระบุความบกพร่องของความสามารถทางกาย ผลการศึกษาช่วยยืนยันการประยุกต์ใช้ระยะจากผนังในการประเมินและติดตามความรุนแรงของภาวะกระดูกสันหลังค่อมในคลินิกและชุมชนต่างๆ

Background and Objectives : The assessment of kyphosis using occiput-wall distance (OWD) is commonly used in epidemiologic studies. However, there were no reports on validity of the method as compared to a standard measurement. Thus this study investigated the correlation of kyphotic measures using OWD and a standard method using flexicurve. Moreover, the study compared the differences of functional capacity in elderly with different severity of kyphosis as determined by using OWD.

Methods : The study was cross-sectionally conducted in elderly, aged at least 60 years old. Sixty-nine subjects were classified into three groups according to severity of kyphosis using data from OWD. Every subject was assessed kyphosis using OWD and flexicurve, and evaluated functional capacity using the 6-minute walk test.

Results : Results of the study demonstrated that a kyphotic measure using the OWD had excellent correlation with flexicurve (r = 0.925, p<0.001). In addition, distance walk in 6 minutes of the subjects were significantly different among the group (p<0.005).

Conclusion : Kyphosis measure using OWD is valid and effective to indicate the impairments of physical ability. The findings confirm the application of OWD to assess and monitor severity of kyphosis both in clinics and communities.
