Clinical Characteristics and outcomes of Treatment of Hodgkin’s Lymphoma

Abstract

Background and objective: Hodgkin’s lymphoma is an uncommon malignancy involving lymph nodes and lymphatic system. Most patients are males and diagnosed between 15-30 years-old, and other peak is more than 50 years-old. Combined modality treatments are the most effective treatment including chemotherapy, radiation, and autologous stem cell transplantation. Currently in Thailand, there is no study of factor that affecting in complete remission (CR) rate, and outcome of treatment. This study aimed to study of treatment outcomes and clinical characteristics that affecting in CR rate of Hodgkin’s lymphoma patients in Khon Kaen hospital.

Method: This cross sectional analytical. The study period was between 2006 and 2016.

Result:  There were 64 patients included in the study. Of those  56.2% were male. The mean (SD) age was 40.72 years. Mixed cellularity was found as the highest  incidence 47.5% and stage 3B for 20.6%. 74.6% of patients had complete remission after complete treatment.  The 5 years overall survival rate was 58% and relapsed rate was only 8.5%.

Conclusion: Factor related to CR rate is Hemoglobin level > 10.5 g/dL and albumin > 4 g/dL statistical significant (p = 0.02)

คุณลักษณะทางคลินิกและผลการรักษาโรคมะเร็งต่อมน้ำเหลืองชนิดฮอดจ์กิ้น

หลักการและวัตถุประสงค์: โรคมะเร็งต่อมน้ำเหลืองชนิดฮอดจ์กิน (Hodgkin's lymphoma) เป็นโรคที่เกิดจากความผิดปกติของต่อมน้ำเหลืองและระบบน้ำเหลืองในร่างกาย ในวัยผู้ใหญ่มักพบในเพศชาย และใน 2 ช่วงอายุ คือ 15-30 ปี และ มากกว่า 50 ปี การรักษาส่วนใหญ่ใช้แบบผสมผสาน ทั้งเคมีบำบัด รังสีรักษาและการปลูกถ่ายไขกระดูก ปัจจุบันในประเทศไทยยังไม่มีการศึกษาถึงผลการรักษา และปัจจัยที่มีความสัมพันธ์กับอัตราการสงบ(complete remissionrate: CRrate) ในผู้ป่วยกลุ่มนี้  ดังนั้นการศึกษานี้จึงมีวัตถุประสงค์เพื่อศึกษาผลการรักษาและปัจจัยที่มีความสัมพันธ์กับ CRrate ของผู้ป่วยHodgkin's lymphoma ในโรงพยาบาลศูนย์ขอนแก่น

วิธีการศึกษา: การศึกษานี้เป็นแบบ Cross sectional analytical study โดยเก็บข้อมูลย้อนหลังจากเวชระเบียนและฐานข้อมูลผู้ป่วย Hodgkin's lymphoma ในช่วงปี พ.ศ. 2549-2559

ผลการศึกษา: พบว่ามีผู้ป่วยจำนวน 64 ราย เป็นเพศชาย ร้อยละ 56.2 พบชนิด Mixed-cellularity เป็นส่วนใหญ่ร้อยละ 47.5 พบระยะ stage 3B ร้อยละ 20.6 ผลการรักษา พบว่าผู้ป่วยร้อยละ 74.6 ได้ CR หลังสิ้นสุดการรักษา มีอัตราการรอดชีวิตที่ 5 ปี ร้อยละ 58 และอัตราการเป็นโรคกลับ (relapse) เพียงร้อยละ 8.5  ปัจจัยที่สัมพันธ์กับ CR rate ได้แก่ค่า Hemoglobin ที่มากกว่า 10.5 g/dL และค่า Albumin ที่มากกว่า 4 g/dL
