Development of Pre-Dispensing System in Out-Patient Department at Sawanpracharak Hospital, Nakhonsawan Province

Background and objectives: Without a protocol working system , the pre-dispensing medication error has been the most common problem in outpatient pharmaceutical service at Sawanpracharak Hospital then, this study aimed to develop the pre-dispensing medication system for the outpatient service implementing an effectiveness protocol and practicing of personnel guidelines leading to patient medication safety usage.
Methods: This study was an action-based research including 3 phases, the first and second phases were done during October 2019 - November 2020 to analyze and develop the protocol of the pre-dispensing medication process. The third phase was done during December 2020 - September 2021 aiming to implement the pre-dispensing medication process and evaluating the results. The data were collected in an official hours. 8,057 prescriptions with errors were found. The comparison between pre- and post- implementing protocol were made by an independent t-test.
Results: The study found that after the development of out-patient pharmaceutical service system. To compare between pre-dispensing medication errors (times per 1,000 prescriptions) before and after implementing protocol: packing errors of 24.4 times were reduced to 10.3 times, input data errors of 15.7 times reduced to 8.3 times, and first checking errors of 10.5 times were reduced to 3 times. The pre-and post- implementing protocols were significantly difference (p< 0.05).
Conclusions: Development of a pre-dispensing medication system for outpatient pharmaceutical services can reduce the rate of medication errors.
