A Comparative Study on the Efficacy of Budesonide versus Saline Nasal Irrigation  in Post Endoscopic Sinus Surgery of Patients with Rhinosinusitis

บทคัดย่อ

หลักการและวัตถุประสงค์: การล้างจมูกด้วยน้ำเกลือเป็นสิ่งจำเป็นในผู้ป่วยไซนัสอักเสบ แต่การศึกษาเปรียบเทียบการล้างจมูกด้วยน้ำเกลือและการล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์บูเดโซไนด์ในประเทศไทยยังมีน้อย ประโยชน์ของการล้างจมูกด้วยสเตียรอยด์ยังไม่ชัดเจนเมื่อเทียบกับการให้น้ำเกลือเพียงอย่างเดียว ดังนั้นการศึกษานี้จึงมีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิภาพการล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์บูเดโซไนด์กับน้ำเกลือหลังการผ่าตัดส่องกล้องของผู้ป่วยไซนัสอักเสบ

วิธีการศึกษา: กลุ่มตัวอย่างผู้ป่วยไซนัสอักเสบที่ได้รับการผ่าตัดด้วยการส่องกล้อง 66 ราย แบ่งเป็น 2 กลุ่ม กลุ่มละ 33 ราย กลุ่มทดลองล้างจมูกด้วยน้ำเกลือ กลุ่มควบคุมล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์
บูเดโซไนด์ ผู้ป่วยทุกรายจะได้รับการประเมินการลดความรุนแรงของไซนัสอักเสบ (the lund–kennedy endoscopic score ; LKES) การลดความรุนแรงของอาการทางจมูก(the symptom visual analog scale ; VAS) และการลดระดับอาการและผลกระทบทางสังคม/อารมณ์(Thai sinonasal outcome test-22; Thai SNOT-22) ก่อนการผ่าตัดและหลังผ่าตัดในสัปดาห์ที่ 2, 4 และ 12 สัปดาห์ วิเคราะห์ข้อมูลด้วย pearson chi-square test, Student t-test และ Wilcoxon rank-sum test

ผลการศึกษา: หลังผ่าตัด 2, 4 และ 12 สัปดาห์ กลุ่มล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์บูเดโซไนด์สามารถลดความรุนแรงของไซนัสอักเสบ (LKES) ได้มากกว่ากลุ่มล้างจมูกด้วยน้ำเกลืออย่างมีนัยสำคัญทางสถิติ (p=0.005, p=0.010, p=0.009) หลังผ่าตัด 12 สัปดาห์ พบว่าในกลุ่มล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์บูเดโซไนด์สามารถลดความรุนแรงริดสีดวงจมูก (polyp) และอาการบวม (edema) ได้มากกว่ากลุ่มล้างจมูกด้วยน้ำเกลืออย่างมีนัยสำคัญทางสถิติ (p=0.001, p=0.022) และยังพบว่าหลังผ่าตัด 12 สัปดาห์ระดับความรุนแรงอาการทางจมูกในกลุ่มล้างจมุกด้วยน้ำเกลือเทียบกับล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์บูเดโซไนด์แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.001, p=0.001) ดังนี้ มัธยฐาน VAS เท่ากับ 7 (IQR 3-9) และ 3 (IQR 1-5) มัธยฐาน Thai SNOT-22 เท่ากับ 20 (IQR 10-24) และ 11 (IQR 7-15) ตามลำดับ

สรุป: การล้างจมูกด้วยน้ำเกลือผสมยาสเตียรอยด์บูเดโซไนด์สามารถลดความรุนแรงอาการทางจมูก เช่น อาการริดสีดวงจมูกและอาการบวม มากกว่าการล้างจมูกด้วยน้ำเกลืออย่างเดียว
