ปัจจัยที่เกี่ยวข้องกับความร่วมมือในการใช้ยาของผู้ป่วยจิตเภท โรงพยาบาลดงหลวง

หลักการและวัตถุประสงค์: การรับประทานยาอย่างต่อเนื่องในผู้ป่วยจิตเภทมีความสำคัญมากในการรักษาและควบคุมอาการผู้ป่วย การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อประเมินความร่วมมือในการใช้ยาและปัจจัยที่มีผลต่อความร่วมมือในการใช้ยาในผู้ป่วยจิตเภท
วิธีการศึกษา: เป็นการศึกษาเชิงพรรณนาแบบภาคตัดขวาง ในผู้ป่วยนอกคลินิกจิตเภท โรงพยาบาลดงหลวง จำนวน 100 รายในช่วง ระหว่างวันที่ 1 กรกฎาคม ถึง 31 ธันวาคม 2560 เครื่องมือที่ใช้ในการศึกษาครั้งนี้ประกอบด้วย 1) แบบวัดความร่วมมือในการรักษาด้วยยาของผู้ป่วยจิตเภทโดยการสัมภาษณ์ผู้ป่วย (MMAS 8–Items) 2) แบบบันทึกการนับเม็ดยา 3) แบบบันทึกข้อมูลส่วนบุคคลและ 4) แบบสัมภาษณ์กึ่งโครงสร้าง สอบถามถึงปัจจัยที่เกี่ยวข้องกับความร่วมมือในการใช้ยา วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาวิเคราะห์ความถดถอยลอจิสติกแบบไบนารี (binary logistic regression)
ผลการศึกษา: ผู้ป่วยจิตเภทส่วนใหญ่ให้ความร่วมมือดีในการรักษาด้วยยาคิดเป็นร้อยละ 62 โดยประเมินจากแบบวัด MMAS 8-Items ร่วมกับการนับเม็ดยา และผลการวิเคราะห์ความถดถอยลอจิสติก พบว่าปัจจัยที่มีผลในการทำนายความร่วมมือในการใช้ยาได้อย่างมีนัยสำคัญทางสถิติ มี 2 ปัจจัย คือ การหยั่งรู้ตน และทัศนคติต่อการใช้ยา โดยมีสัมประสิทธิ์การทำนายร้อยละ 22
สรุป: ผู้ป่วยจิตเภทส่วนใหญ่ในโรงพยาบาลดงหลวง (ร้อยละ 62) มีความร่วมมือในการใช้ยาที่ดีเมื่อเปรียบเทียบกับข้อมูลการศึกษาที่ผ่านมาและพบว่าปัจจัยที่สัมพันธ์กับความร่วมมือในการใช้ยา คือ การหยั่งรู้ตน และทัศนคติต่อการใช้ยา
