Prevalence and Factors Associated with Depression among the Clinical Medical Students of a Faculty of Medicine in Northeast Thailand

หลักการและวัตถุประสงค์: ภาวะซึมเศร้าเป็นสาเหตุสำคัญอันดับต้นที่ทำให้เกิดภาวะทุพลภาพ นักศึกษาแพทย์ชั้นคลินิกต้องปฏิบัติงานบนหอผู้ป่วยด้วยนอกเหนือจากการเรียนในภาคบรรยาย ซึ่งก่อให้เกิดความเครียดทั้งทางร่างกายและจิตใจ การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในนักศึกษาแพทย์ชั้นคลินิก
วิธีการศึกษา: รูปแบบการวิจัยเป็นการศึกษาเชิงพรรณนาภาคตัดขวาง แบบสอบถามประกอบด้วยข้อมูลทั่วไปและแบบคัดกรองโรคซึมเศร้า 2 คำถาม และแบบประเมินโรคซึมเศร้า 9 คำถาม ส่งแบบสอบถามอิเล็กทรอนิกส์ให้กับนักศึกษาแพทย์ชั้นคลินิกทุกราย จำนวน 133 ราย ในช่วงเวลา 1 – 31 กรกฎาคม 2562 วิเคราะห์ข้อมูลด้วยโปรแกรม SPSS Statistics version 22 ประกอบด้วย สถิติเชิงพรรณนา ได้แก่ จำนวน ร้อยละ ค่าสูงสุด ค่าต่ำสุด ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่ามัธยฐาน และค่าพิสัยควอไทล์ และสถิติเชิงวิเคราะห์ Chi-square test และ Fisher’s exact test
ผลการศึกษา: มีผู้เข้าร่วมการศึกษา 92 ราย พบความชุกของภาวะซึมเศร้าจำนวน 36 ราย (ร้อยละ 39.1; 95%CI 29.1- 49.9) นักศึกษาแพทย์ชั้นคลินิกส่วนใหญ่มีภาวะซึมเศร้าระดับน้อย 22 ราย (ร้อยละ 23.9) ปัจจัยที่มีความสัมพันธ์กับภาวะซึมเศร้าอย่างมีนัยสำคัญทางสถิติ ได้แก่ นักศึกษาที่พักการศึกษา (p = 0.043)
สรุป: หนึ่งในสามของนักศึกษาแพทย์ชั้นคลินิกมีภาวะซึมเศร้า ควรให้ความสำคัญการคัดกรองและติดตามภาวะซึมเศร้าในนักศึกษาแพทย์ชั้นคลินิกโดยเฉพาะนักศึกษาแพทย์ชั้นคลินิกที่พักการศึกษา
