บทคัดย่อ

ภูมิหลังและวัตถุประสงค์: แบบประเมิน Functional Assessment Staging Test (FAST) เป็นเครื่องมือมาตรฐานสากลสำหรับประเมินระดับความรุนแรงของโรคอัลไซเมอร์ซึ่งเป็นสาเหตุที่พบบ่อยที่สุดของโรคสมองเสื่อม แบ่งระดับความรุนแรงของโรคเป็น 7 ระยะหรือ 16 ระดับย่อย การวิจัยนี้มีวัตถุประสงค์เพื่อแปลและตรวจสอบความเที่ยงตรงเชิงเนื้อหาและความเชื่อมั่นระหว่างผู้ประเมินของแบบประเมิน FAST ฉบับภาษาไทย

วิธีการศึกษา: ดำเนินการตามแนวทาง ISPOR ประกอบด้วยการแปลไปข้างหน้า การตรวจสอบโดยผู้เชี่ยวชาญ 3 ท่าน การแปลย้อนกลับโดยผู้ทรงคุณวุฒิ 2 ท่าน และการทดสอบความเชื่อมั่นระหว่างผู้ประเมิน 2 ท่านในผู้ป่วยสมองเสื่อม 30 ราย ณ โรงพยาบาลศรีนครินทร์ ช่วงเดือนสิงหาคม พ.ศ. 2567 ถึงมกราคม พ.ศ. 2568 วิเคราะห์ด้วยค่า Content Validity Index (CVI) และ Intraclass Correlation Coefficient (ICC)

ผลการศึกษา: แบบประเมิน FAST ฉบับภาษาไทยมีค่า CVI = 0.89 และ ICC = 0.995 (95%CI: 0.990-0.997, p < 0.001) ผู้ป่วยส่วนใหญ่อยู่ใน FAST ระยะ 6-7 (ร้อยละ 90.0)

สรุป: แบบประเมิน FAST ฉบับภาษาไทยมีความเที่ยงตรงเชิงเนื้อหาและความเชื่อมั่นระหว่างผู้ประเมินในระดับดีเยี่ยม เหมาะสมสำหรับการประเมินระยะของโรคสมองเสื่อมในบริบทไทย โดยเฉพาะในผู้ป่วยระยะท้าย เพื่อประโยชน์ในการพยากรณ์โรค วางแผนการดูแลรักษา และส่งต่อเข้าสู่การดูแลแบบประคับประคองได้อย่างทันท่วงที
