การพัฒนารูปแบบการดำเนินงานเพื่อส่งเสริมการดูแลโรคเบาหวานให้เข้าสู่ระยะสงบ (diabetes remission): ผลลัพธ์เชิงระบบของเครือข่ายสุขภาพจังหวัดร้อยเอ็ด

ภูมิหลังและวัตถุประสงค์: โรคเบาหวานเป็นภาวะเรื้อรังที่เกิดจากร่างกายไม่สามารถควบคุมระดับน้ำตาลในเลือดได้อย่างมีประสิทธิภาพ ซึ่งหากปล่อยไว้เป็นเวลานานอาจส่งผลกระทบต่อระบบต่างๆ ของร่างกายอย่างรุนแรง การศึกษาครั้งมีวัตถุประสงค์เพื่อศึกษาสภาพปัญหา พัฒนาและทดลองใช้ และประเมินผลรูปแบบการดำเนินงานเพื่อส่งเสริมการดูแลโรคเบาหวานให้เข้าสู่ระยะสงบ โดยใช้ปัญหาเป็นฐาน
วิธีการศึกษา: เป็นการศึกษาและพัฒนา กลุ่มตัวอย่างเป็นทีมสหสาขาวิชาชีพและอาสาสมัครสาธารณสุข 220 ราย และผู้ป่วยโรคเบาหวานที่ผู้เข้าร่วมโครงการ 4,031 ราย เก็บข้อมูลโดยใช้แบบสอบถาม แบบบันทึก และแนวคำถามสำหรับการสัมภาษณ์ วิเคราะห์ข้อมูลโดยใช้ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน paired t-test พร้อม 95% confidence interval และการวิเคราะห์เชิงเนื้อหา
ผลการศึกษา: พบว่า 1) สภาพปัญหาการดำเนินงานการดูแลโรคเบาหวานในจังหวัดร้อยเอ็ดจำแนกเป็นปัญหาด้านผู้ป่วยและบริบททางสังคม ผู้ให้บริการ ระบบบริหารจัดการบริการ และทรัพยากรและเวชภัณฑ์ 2) รูปแบบการดำเนินงานเพื่อส่งเสริมการดูแลโรคเบาหวานให้เข้าสู่ระยะสงบ โดยใช้ปัญหาเป็นฐาน ประกอบด้วยกิจกรรมการคัดกรองคนที่ "ใช่" การตั้งเป้าหมายร่วมกัน โรงเรียนฝึกทักษะ การใช้เทคโนโลยีช่วยตามผล ทีมดูแลและการปรับลดยา การสร้างกำลังใจและเครือข่าย และการติดตามผลอย่างใกล้ชิด และ 3) หลังการพัฒนา พบว่า ผู้ป่วยมีคะแนนเฉลี่ยพฤติกรรมการดูแลตนเองสูงขึ้นกว่าก่อนการพัฒนา (p<0.001) ทุกด้าน และกลุ่ม diabetes remission success จากกลุ่มที่ครบระยะประเมินผล 3 เดือน (2,261 ราย) พบผู้ป่วยที่สามารถบรรลุสภาวะโรคสงบคือ สามารถหยุดยาได้ต่อเนื่องอย่างน้อย 3 เดือน และมีระดับ HbA1c < 6.5 mg% จำนวน 1,004 ราย (ร้อยละ 24.91 ของผู้เข้าร่วมโครงการ) กลุ่ม medication reduction นอกจากกลุ่มที่บรรลุสภาวะโรคสงบแล้ว ยังมีกลุ่มที่มีทิศทางดีขึ้น โดยมีผู้ป่วยที่สามารถลดจำนวนรายการยาลงได้ ร้อยละ 30.22 (1,218 ราย) และกลุ่ม clinical stability & safety ผู้ป่วยส่วนใหญ่รักษาระดับการใช้ยาคงที่ (ร้อยละ 30.79) ในขณะที่มีอัตราการเพิ่มยาเพียงร้อยละ 0.45 และมีอัตราการขาดการติดตามเพียงร้อยละ 0.27
สรุป: การดูแลโรคเบาหวานสู่ระยะสงบโดยใช้การฝึกทักษะและการติดตามผลอย่างใกล้ชิดจนผู้ป่วยมีพฤติกรรมดูแลตนเองดีขึ้น ส่งผลให้ผู้ป่วยบรรลุสภาวะโรคสงบและสามารถลดการใช้ยาลงได้
