บทคัดย่อ

ภูมิหลังและวัตถุประสงค์: ต้อกระจกเป็นสาเหตุสำคัญของตาบอดที่สามารถรักษาได้ การติดเชื้อในลูกตาหลังผ่าตัดแม้พบไม่บ่อยแต่มีความรุนแรง การศึกษานี้จึงมีวัตถุประสงค์เพื่อประเมินความปลอดภัยและผลลัพธ์ทางคลินิกของการใช้สารละลายไอโอดีนความเข้มข้น 0.25% ระหว่างการผ่าตัดต้อกระจกเพื่อป้องกันการติดเชื้อในลูกตาหลังผ่าตัด

วิธีการศึกษา: การศึกษาย้อนหลังจากเวชระเบียนผู้ป่วยที่ผ่าตัดต้อกระจก ช่วงวันที่ 1 มกราคม พ.ศ. 2564 ถึง 30 มีนาคม พ.ศ. 2568 ที่ได้รับการหยอดสารละลายไอโอดีนความเข้มข้น 0.25% ระหว่างการผ่าตัด วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา

ผลการศึกษา: ผู้ป่วย 838 ราย อายุเฉลี่ย 66.9 ± 9.6 ปี ส่วนใหญ่เป็นเพศหญิง (ร้อยละ 56.9) การผ่าตัดเป็นวิธีสลายเลนส์ด้วยคลื่นเสียงความถี่สูงร่วมกับการใส่เลนส์แก้วตาเทียม (ร้อยละ 97.6) วันแรกหลังผ่าตัดพบการอักเสบในช่องหน้าลูกตาระดับ 1+, 2+ และ 3+ร้อยละ 28.6, 60.0 และ 11.4 ตามลำดับ และลดลงจนเหลือระดับ trace ร้อยละ 99.5 ภายใน 7 วัน มีระยะเวลาเฉลี่ยที่ไม่พบการอักเสบ 32.9 ± 6.9 วัน ไม่พบการติดเชื้อในลูกตาหลังผ่าตัดตลอดระยะเวลาติดตามเฉลี่ย 9.9 ± 6.0 เดือน การมองเห็นหลังผ่าตัดดีกว่าหรือเท่ากับ 6/12 ร้อยละ 77 หลังการผ่าตัดเดือนที่ 6

สรุป: การใช้สารละลายไอโอดีนความเข้มข้น 0.25% ระหว่างการผ่าตัดต้อกระจก แสดงให้เห็นถึงความปลอดภัยและให้ผลลัพธ์ทางคลินิกในระดับที่ยอมรับได้ โดยไม่พบการติดเชื้อในช่วงติดตาม อย่างไรก็ตาม การศึกษานี้ไม่ได้มีกลุ่มเปรียบเทียบ จึงไม่สามารถแสดงประสิทธิภาพในการป้องกันภาวะการติดเชื้อที่แตกต่างกับการไม่ใช้ หรือการใช้วิธีอื่นได้
