ความรู้ พฤติกรรมการบริโภค และสภาวะสุุขภาพของผู้ป่วยฟอกเลือด ภายหลังการใช้โมเดลการจัดการตนเองผ่านแอปพลิเคชัน

ภูมิหลังและวัตถุประสงค์: การจัดการตนเองของผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือด มีความสำคัญต่อการควบคุมสมดุลสารน้ำและอิเล็กโตรไลท์ อย่างไรก็ตาม การให้ความรู้ในหน่วยไตเทียมมักเป็นลักษณะครั้งคราวและอาจไม่เพียงพอ ในการสนับสนุนการปรับเปลี่ยนพฤติกรรมอย่างต่อเนื่อง ดังนั้น การศึกษานี้จึงมีวัตถุประสงค์เพื่อประเมินการเปลี่ยนแปลงของความรู้ พฤติกรรมการบริโภค และตัวชี้วัดทางคลินิก ได้แก่ ความดันโลหิตและระดับอิเล็กโตรไลท์ ภายหลังการใช้โมเดลการจัดการตนเองแบบบูรณาการผ่านแอปพลิเคชันในผู้ป่วยฟอกเลือด
วิธีการศึกษา: เป็นการศึกษากึ่งทดลองแบบกลุ่มเดียว วัดผลก่อนและหลังการแทรกแซง ดำเนินการในหน่วยไตเทียม 5 แห่ง จังหวัดมหาสารคาม ผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายจำนวน 99 ราย ได้รับการสนับสนุนการจัดการตนเองผ่านแอปพลิเคชัน ซึ่งประกอบด้วยการให้ความรู้ การติดตามพฤติกรรม และการสื่อสารสองทาง เป็นระยะเวลา 4 สัปดาห์ วิเคราะห์ความแตกต่างก่อนและหลังโดยใช้สถิติ paired t-test
ผลการศึกษา: ภายหลังการแทรกแซง พบว่าคะแนนความรู้ด้านสารน้ำและโภชนาการเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p<0.001) จาก 8.43±1.67 เป็น 9.11±1.22 (p<0.001) และจาก 7.23±1.67 เป็น 8.44±1.22 (p<0.001) ตามลำดับ คะแนนพฤติกรรมการดูแลตนเองโดยรวมเพิ่มขึ้นจาก 10.34±2.29 เป็น 13.36±1.64 (p<0.001) และความถี่ของการบริโภคอาหารเค็มในมื้อเช้าและกลางวันลดลงอย่างมีนัยสำคัญ จาก 8.40±18.10 เป็น 5.00±14.40 และ 13.90±23.00 เป็น 8.10±17.90 อย่างไรก็ตาม ไม่พบความแตกต่างอย่างมีนัยสำคัญในค่าความดันโลหิตและระดับอิเล็กโตรไลท์ในเลือด โดยค่าทางคลินิกยังคงอยู่ในช่วงที่ยอมรับได้
สรุป: การใช้รูปแบบการจัดการตนเองผ่านแอปพลิเคชัน มีประสิทธิภาพในการพัฒนาความรู้และพฤติกรรมการดูแลตนเองในผู้ป่วยฟอกเลือด อย่างไรก็ตาม ไม่พบการเปลี่ยนแปลงของตัวชี้วัดทางคลินิกในระยะสั้น ผลลัพธ์ดังกล่าวสะท้อนศักยภาพของการใช้เทคโนโลยีดิจิทัล ในการสนับสนุนการดูแลต่อเนื่องในบริบทจริง ทั้งนี้ ควรมีการศึกษาเพิ่มเติมที่มีกลุ่มเปรียบเทียบและระยะเวลาติดตามที่ยาวนานขึ้นเพื่อยืนยันผลลัพธ์
