การพัฒนารูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองระยะฟื้นฟูสภาพ โรงพยาบาลกระแสสินธุ์

การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม เพื่อวิเคราะห์สถานการณ์และพัฒนารูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองระยะฟื้นฟูสภาพ และเพื่อประเมินผลการใช้รูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองระยะฟื้นฟูสภาพ โรงพยาบาลกระแสสินธุ์ มี 4 ระยะ คือ ระยะที่ 1 วิเคราะห์สถานการณ์ ระยะที่ 2 พัฒนารูปแบบ ระยะที่ 3 นำรูปแบบไปใช้ และระยะที่ 4 ประเมินผลการใช้รูปแบบ เลือกกลุ่มตัวอย่างแบบเจาะจง กลุ่มตัวอย่าง 3 กลุ่ม คือ ผู้ป่วย จำนวน 5 คน ผู้ดูแล จำนวน 5 คน ผู้ให้การดูแลผู้ป่วย จำนวน 15 คน รวม 25 คน และผู้ป่วยโรคหลอดเลือดสมองรายใหม่ จำนวน 30 คน เลือกกลุ่มตัวอย่างแบบเจาะจงจากผู้ป่วยรายใหม่ ตามเกณฑ์คัดเข้า คัดออก จำนวน 10 คน เครื่องมือที่ใช้วิจัย คือ แนวคำถามในการสนทนากลุ่ม และแนวคำถามการสัมภาษณ์เชิงลึก ตรวจสอบความตรงของเนื้อหา จากผู้เชี่ยวชาญ 3 ท่าน ได้ค่าความตรงของเนื้อหา ส่วนที่ 1 เท่ากับ 0.88, ส่วนที่ 4 เท่ากับ 0.82 เก็บข้อมูลด้วยการสนทนากลุ่มและการสัมภาษณ์เชิงลึก วิเคราะห์ข้อมูล ค่าร้อยละ ค่าเฉลี่ย และวิเคราะห์เชิงเนื้อหา
ผลการศึกษา พบว่า รูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองระยะฟื้นฟูสภาพ ประกอบด้วย การรับผู้ป่วย วิธีการเยี่ยมบ้าน ทีมสหวิชาชีพ และการดูแลรักษาต่อเนื่อง ผู้ป่วยรายใหม่ได้รับการเยี่ยมบ้านทุกราย ทีมเยี่ยมบ้านมีความพึงพอใจมากที่สุด ร้อยละ 91.78 รูปแบบมีความชัดเจนเข้าใจง่ายนำลงสู่การปฏิบัติได้จริง ส่งผลให้ผู้ป่วยโรคหลอดเลือดสมองได้รับการฟื้นฟูสภาพรวดเร็วขึ้น
ข้อเสนอแนะ ควรนำรูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองระยะฟื้นฟูสภาพไปใช้เพื่อส่งเสริมให้ผู้ป่วยโรคหลอดเลือดสมองได้รับการฟื้นฟูสภาพ และมีคุณภาพชีวิตที่ดีขึ้น
