รายงานสัตว์ป่วย: มะเร็งผิวหนังชนิดเมลาโนมาที่พบในช่องปากบริเวณเหงือกขากรรไกรล่างขวาในสุนัข

บทคัดย่อ
สุนัขพันธุ์ผสม เพศผู้ ทำหมันแล้ว อายุ 12 ปี ตรวจพบก้อนเนื้อภายในช่องปากบริเวณเหงือกขากรรไกรล่างด้านขวาเป็นมาประมาณ 2 เดือน (ขนาด 4x5 เซนติเมตร) โดยมีประวัติเข้ารับการผ่าตัดมะเร็งผิวหนังชนิดเมลาโนมาในช่องปาก (oral malignant melanoma) ออกจากกรามล่างด้านขวามาแล้วประมาณ 1 ปี การตรวจร่างกายเบื้องต้นไม่พบความผิดปกติอื่น นอกจากพบการขยายขนาดของต่อมน้ำเหลืองบริเวณใต้ขากรรไกรล่างด้านขวา (right submandibular lymph node) ต่อมน้ำเหลืองบริเวณใต้ขากรรไกรล่างด้านซ้าย (left submandibular lymph node) และต่อมน้ำเหลืองที่ขาพับหลังด้านขวา (right popliteal lymph node) ผลตรวจทางจุลพยาธิวิทยาวินิจฉัยยืนยันว่าสุนัขป่วยเป็นมะเร็งผิวหนังชนิดเมลาโนมาและพบการแพร่กระจายของมะเร็งไปยังต่อมน้ำเหลืองใต้ขากรรไกรล่างด้านขวา สุนัขได้รับการรักษาโดยการผ่าตัดกรามล่างด้านขวาออกทั้งหมด (total right hemimandibulectomy) ร่วมกับการใช้เคมีบำบัดภายหลังการศัลยกรรม โดยเลือกใช้โลมัสทีน (Lomustine, CCNU) เป็นระยะเวลานาน 18 สัปดาห์ ผลการรักษาพบต่อมน้ำเหลืองขนาดเล็กลงและไม่พบก้อนเนื้อกลับมาในตำแหน่งเดิมและบริเวณใกล้เคียง ผลการถ่ายภาพรังสีช่องอกและการตรวจคลื่นเสียงความถี่สูงของอวัยวะในช่องท้องไม่พบการแพร่กระจายของเซลล์มะเร็งเมื่อเทียบกับก่อนทำเคมีบำบัด
