Development of Palliative Care Model at Tha Bo Crown Prince Hospital in Nong Khai Province

This study aimed to investigate the situation as well as develop model and evaluate the effectiveness of palliative care model at Tha Bo Crown Prince Hospital. The process of study consisted of; 1) situation analysis of problem by using qualitative research. The instrument of this phase was question guideline. Data were collected by focus group discussion, 2) the SGHAR model was used to develop a palliative care model (which included service access, patient care guidelines, holistic care, advanced care planning, and data referral to the community), and a hospital palliative care booklet (which included symptom screening, caring and referring patient guideline, primary care for common symptoms, symptom assessment, medication administration, epilogue) was created. Samples of this study were patients with advanced cancer, primary caregivers, and nurses. The instruments included Edmonton Symptom Assessment System (ESAS), and a satisfaction assessment of family caregivers and nurses. Data were analyzed by using content analysis and descriptive statistics. The results revealed as follows:
- The palliative care situation revealed that nursing professions lacked knowledge and understanding of palliative care, different guidelines were used in each ward, the psychological and spiritual aspects of caring were not covered, and patients and family caregivers lacked knowledge of the disease.
- The effectiveness of using the SGHAR Model and the caring booklet revealed that patients' discomfort symptoms, particularly pain, anxiety, and fatigue, were decreased. Family caregivers had the highest level of satisfaction (70%) while nurses had a high level of satisfaction (60%)
