การพัฒนารูปแบบการดูแลผู้ป่วยระยะท้ายแบบประคับประคอง โรงพยาบาลสมเด็จพระยุพราชท่าบ่อ จังหวัดหนองคาย

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาสถานการณ์ รวมทั้งพัฒนารูปแบบและศึกษาประสิทธิผลของการดูแลผู้ป่วยระยะท้ายแบบประคับประคองในโรงพยาบาลสมเด็จพระยุพราชท่าบ่อ โดยมีขั้นตอนดังนี้ 1) ศึกษาสถานการณ์ของปัญหา เป็นการวิจัยเชิงคุณภาพ เครื่องมือที่ใช้เป็นประเด็นคำถาม เก็บข้อมูลโดยการสนทนา 2) พัฒนารูปแบบการดูแลผู้ป่วยระยะท้ายแบบประคับประคองโดยใช้รูปแบบ SGHAR Model (ประกอบด้วย การเข้าถึงบริการ, แนวทางการดูแลผู้ป่วย ,การดูแลแบบองค์รวม, การวางแผนดูแลล่วงหน้า ,การส่งต่อข้อมูลสู่ชุมชน) และจัดทำคู่มือการดูแลผู้ป่วยระยะท้ายแบบประคับประคองประจำโรงพยาบาล (ประกอบด้วย การคัดกรองผู้ป่วยเข้ารับบริการ, แนวปฏิบัติในการดูแลและส่งต่อผู้ป่วย, การดูแลบรรเทาอาการเบื้องต้น, การประเมินอาการ, การใช้ยา, บทส่งท้าย) โดยศึกษาในกลุ่มตัวอย่างคือ ผู้ป่วยมะเร็งระยะท้าย, ผู้ดูแล และพยาบาล เครื่องมือที่ใช้คือ แบบประเมินอาการไม่สุขสบายของผู้ป่วย (ESAS) แบบประเมินความพึงพอใจของผู้ดูแลและความพึงพอใจของพยาบาล แล้ววิเคราะห์ข้อมูลทั้งเชิงเนื้อหาและเชิงปริมาณโดยใช้สถิติเชิงพรรณนา ผลการวิจัยพบว่า
- สถานการณ์การดูแลผู้ป่วยแบบประคับประคองพบว่า บุคลากรทางการพยาบาลยังไม่มีความรู้ความเข้าใจในการดูแลแบบประคับประคองที่เพียงพอ พยาบาลแต่ละตึกดูแลผู้ป่วยไม่เป็นแนวทางเดียวกัน การดูแลไม่ครอบคลุมด้านจิตใจและจิตวิญญาณ ผู้ป่วยและครอบครัวไม่ได้รับข้อมูลเกี่ยวกับโรคและความเจ็บป่วยที่เพียงพอ
- ผลการใช้ SGHAR Model และคู่มือการดูแลผู้ป่วย พบว่า อาการไม่สุขสบายของผู้ป่วยลดลงโดยเฉพาะความปวด ความวิตกกังวล อาการเหนื่อยเพลีย ความพึงพอใจของผู้ดูแลอยู่ในระดับมากที่สุดร้อยละ 70 ความพึงพอใจของพยาบาลอยู่ในระดับมาก ร้อยละ 60
