ผลการพัฒนาแนวปฏิบัติการคัดแยกผู้ป่วยตาม MOPH ED Triage งานอุบัติเหตุและฉุกเฉิน โรงพยาบาลเมืองสงขลา

ความเป็นมาและวัตถุประสงค์ การพัฒนาแนวปฏิบัติการคัดแยกผู้ป่วยให้ชัดเจน เข้าใจง่าย ถือเป็นแนวทางสำคัญที่ช่วยลดความคลาดเคลื่อนและเพิ่มความแม่นยำในการคัดแยกผู้ป่วยได้อย่างมีประสิทธิภาพ การวิจัยครั้งนี้จึงมีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติการคัดแยกผู้ป่วยตามรูปแบบ MOPH ED Triage และประเมินประสิทธิภาพในการยกระดับคุณภาพการบริการในงานอุบัติเหตุและฉุกเฉินของโรงพยาบาลเมืองสงขลา
วิธีการวิจัย การวิจัยนี้เป็นการวิจัยเชิงปฏิบัติการ (Action Research) แบบกลุ่มเดียววัดก่อนและหลังการใช้แนวปฏิบัติ (one-group pretest–posttest design) ดำเนินการ 4 ระยะ คือ 1) Plan ศึกษาปัญหาและสถานการณ์ก่อนใช้แนวปฏิบัติจากบุคลากรทางการแพทย์ผู้ทำหน้าที่คัดแยกผู้ป่วย 20 คน และเวชระเบียน 427 คน 2) Act พัฒนาแนวปฏิบัติ และฝึกอบรมบุคลากร 3) Observe ประเมินผลหลังใช้แนวปฏิบัติจากบุคลากร 20 คน และเวชระเบียน 434 คน ข้อมูลถูกวิเคราะห์ด้วยสถิติเชิงพรรณนา และการทดสอบทีอิสระ และ 4) Reflect วิเคราะห์จุดแข็ง จุดที่ควรปรับปรุง และแนวทางพัฒนาของแนวปฏิบัติ
ผลการวิจัย แนวปฏิบัติที่พัฒนาขึ้นช่วยเพิ่มความถูกต้องในการคัดแยกผู้ป่วยจากร้อยละ 79.40 เป็นร้อยละ 88.70 นอกจากนี้ยังพบว่าระยะเวลารอรับการรักษาของผู้ป่วยทุกกลุ่มลดลงอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ด้านความพึงพอใจของบุคลากรพบว่า แนวปฏิบัติได้รับการประเมินในระดับสูง โดยเฉพาะในด้านความถูกต้อง ความสามารถในการนำไปใช้ได้จริง และประโยชน์ต่อผู้ป่วยและหน่วยงาน
สรุปและข้อเสนอแนะ ผลการศึกษาสามารถใช้เป็นแนวทางการพัฒนาและปรับปรุงแนวปฏิบัติการคัดแยกผู้ป่วยในงานอุบัติเหตุฉุกเฉิน และสามารถยกระดับการให้บริการทางการแพทย์ได้อย่างมีประสิทธิภาพ
