ผลของโปรแกรมการพยาบาลจัดการรายกรณีสำ�หรับผู้ป่วยระยะท้ายต่อการรับรู้ คุณภาพการดูแลแบบประคับประคองและความพึงพอใจของผู้ดูแลหลัก

การวิจัยกลุ่มเดียววัดก่อนและหลัง มีวัตถุประสงค์เพื่อศึกษาการรับรู้คุณภาพการดูแลก่อนและหลังได้รับโปรแกรมการพยาบาลการจัดการรายกรณีสำหรับผู้ป่วยระยะท้ายและความพึงพอใจของผู้ดูแลหลักหลังได้รับโปรแกรม การคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง กำหนดคุณสมบัติผู้ดูแลหลักดังนี้ 1) เป็นผู้รับผิดชอบหลักในการดูแลผู้ป่วยระยะท้ายโดยตรง2) รับทราบจากแพทย์ว่า ผู้ป่วยอยู่ในระยะท้ายและให้การดูแลแบบประคับประคอง และ 3) สามารถสื่อสารฟัง อ่าน เขียนภาษาไทยได้ในการให้ข้อมูลจำนวน 30 คน เข้าร่วมโปรแกรมการพยาบาลการจัดการรายกรณีสำหรับผู้ป่วยระยะท้ายเป็นระยะเวลา 5 วัน ประกอบด้วย 5 ขั้นตอนดังนี้ 1) ค้นหาและเลือกผู้ป่วย 2) ประเมินและวินิจฉัยปัญหา 3) วางแผนการดูแลที่จำเป็นสำหรับผู้ป่วยเฉพาะรายตามปัญหาที่พบ 4) ดำเนินการตามแผน และ 5) ประเมินผลลัพธ์ของการจัดการรายกรณี เก็บรวบรวมข้อมูลโดยบันทึกข้อมูลทั่วไปของผู้ป่วยและผู้ดูแลหลัก แบบประเมินการรับรู้คุณภาพการดูแล โดยใช้แบบสอบถามผลลัพธ์การดูแลแบบประคับประคอง และความพึงพอใจในบริการพยาบาล วิเคราะห์ความเชื่อมั่นของแบบสอบถามผลลัพธ์การดูแลแบบประคับประคอง และความพึงพอใจของผู้ดูแลหลักต่อรูปแบบการจัดการรายกรณีสำหรับการดูแลผู้ป่วยแบบประคับประคองโดยหาค่าสัมประสิทธิ์แอลฟ่าของครอนบาค ได้เท่ากับ .82 และ .90 ตามลำดับ วิเคราะห์ข้อมูลทั่วไปและความพึงพอใจในบริการพยาบาลด้วยสถิติบรรยาย เปรียบเทียบการรับรู้คุณภาพการดูแลแบบประคับประคอง ด้วยการวิเคราะห์สถิติทีคู่
ผลการวิจัย พบว่า หลังการทดลอง กลุ่มตัวอย่างรับรู้คุณภาพการดูแลสูงกว่าก่อนการดูแลอย่างมีนัยสำคัญทางสถิติ
(t = 5.43, p < 0.001) และความพึงพอใจของผู้ดูแลหลักต่อโปรแกรมการพยาบาลการจัดการรายกรณีสำหรับผู้ดูแลผู้ป่วย
ระยะท้ายอยู่ในระดับสูง ( = 4.03, S.D. = .67) ดังนั้นหน่วยงานที่มีศูนย์การดูแลแบบประคับประคองควรมีการนำโปรแกรม
การพยาบาลจัดการรายกรณีสำหรับผู้ป่วยระยะท้ายไปใช้เพื่อส่งเสริมคุณภาพการดูแล
