บทคัดย่อ

Stenotrophomonas maltophilia เป็นสาเหตุของการติดเชื้อในโรงพยาบาลและมีอัตราการตายสูงขึ้นเนื่องจากดื้อต่อยาต้านจุลชีพหลายชนิดทำให้ยากต่อการรักษา งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยเสี่ยงที่มีผลต่อการตายและยาปฏิชีวนะที่ใช้ในการรักษาของผู้ป่วยที่ติดเชื้อสตีโนโทรโฟโมแนสมัลโตฟีลเลีย โดยเก็บข้อมูลจากเวชระเบียนย้อนหลังตั้งแต่มิถุนายน 2559 ถึงมิถุนายน 2562 วิเคราะห์ข้อมูลใช้โปรแกรม SPSS รายงานความสัมพันธ์ด้วย Odd ratio และวิเคราะห์ความสัมพันธ์ของตัวแปรโดยวิธี Multivariate logistic regression โดยมีความแตกต่างอย่างมีนัยสำคัญทางสถิติเมื่อ p value < 0.05 พบว่า ผู้ป่วยมีจำนวนทั้งหมด 404 ราย อายุเฉลี่ย 60.6 ปี เพศชาย (67.8%) พบการติดเชื้อในระบบทางเดินหายใจ (81.9%), ระบบผิวหนังและเนื้อเยื่อ (5.7%) และระบบทางเดินปัสสาวะ (4.5%) ตามลำดับ เชื้อมีความไวต่อยา trimethoprim/sulfamethoxazole (90.6%), levofloxacin (87.4%), และ sulbactam/cefoperazone (85.6%) ตามลำดับ อัตราการตาย 19.0% ปัจจัยที่มีความสัมพันธ์กับอัตราตายจากการวิเคราะห์ถดถอยเอกนาม ได้แก่ โรคหัวใจ (P = 0.004) ติดเชื้อเอชไอวี (P = 0.049) ได้รับยากดภูมิคุ้มกัน (P = 0.014) ใส่สายสวนปัสสาวะ (P = 0.005) ใส่ท่อช่วยหายใจ (P < 0.001) ใส่สายสวนหลอดเลือดดำ (P < 0.001) และผ่าตัด (P = 0.001) ปัจจัยที่มีผลต่ออัตราตายจากการวิเคราะห์ถดถอยโลจิสติกพหุนาม ได้แก่ การใส่ท่อช่วยหายใจ (P < 0.001) การใส่สายสวนหลอดเลือดดำ (P = 0.002) และการผ่าตัด (P < 0.001) สรุปได้ว่าการใส่ท่อช่วยหายใจและการใส่สายสวนหลอดเลือดดำเป็นเวลานานมีความสัมพันธ์กับอัตราตายจากการติดเชื้ออย่างมีนัยสำคัญ และเชื้อยังมีความไวต่อยาหลักคือ trimethoprim /sulfamethoxazole และยารองคือ levofloxacin และ sulbactam/cefoperazone
