การพัฒนาแนวปฏิบัติการพยาบาลทางคลินิกเพื่อป้องกันท่อช่วยหายใจเลื่อนหลุด ในหอผู้ป่วยหนักอายุรกรรม โรงพยาบาลมหาสารคาม

วัตถุประสงค์ของการวิจัยเพื่อพัฒนาแนวปฏิบัติการพยาบาลทางคลินิกเพื่อป้องกันท่อช่วยหายใจเลื่อนหลุด ตามรูปแบบการใช้หลักฐานเชิงประจักษ์ในการปฏิบัติของศูนย์ปฏิบัติการพยาบาลขั้นสูง ทำการศึกษา 3 ระยะ ระยะที่ 1 การค้นหาปัญหาทางคลินิก ระยะที่ 2 การสืบค้นหลักฐานเชิงประจักษ์ และ ระยะที่ 3 การทดลองใช้แนวปฏิบัติ ใช้การวิจัยกึ่งทดลอง กลุ่มตัวอย่างเป็นพยาบาล 12 คน และผู้ป่วย 62 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 31 คน เครื่องมือที่ใช้ในการเก็บข้อมูล 1) แบบบันทึกอุบัติการณ์การเกิดท่อช่วยหายใจเลื่อนหลุด และ 2) แบบประเมินความเป็นไปได้ในการนำแนวปฏิบัติไปใช้ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและไคสแควร์
ผลการวิจัย พบว่าแนวปฏิบัติการพยาบาลทางคลินิกเพื่อป้องกันท่อช่วยหายใจเลื่อนหลุด มี 4 ด้าน คือ 1) การประเมินความเสี่ยงของการเกิดท่อช่วยหายใจเลื่อนหลุด 2) การดูแลขณะผู้ป่วยใส่ท่อช่วยหายใจ 3) การให้ข้อมูลการใส่ท่อช่วยหายใจและการสื่อสาร และ 4) การบริหารจัดการ หลังการทดลองพบว่ากลุ่มควบคุมและกลุ่มทดลองมีอุบัติการณ์การเกิดท่อช่วยหายใจเลื่อนหลุด 8.47 ครั้ง และ 4.44 ครั้ง ต่อ 1,000 วันใส่ท่อช่วยหายใจ ตามลำดับ สัดส่วนของการเกิดท่อช่วยหายใจเลื่อนหลุดระหว่างสองกลุ่มไม่แตกต่างกัน (χ2 = .350, p > .05) พยาบาลร้อยละ 100 มีคะแนนความเป็นไปได้ในการนำแนวปฏิบัติไปใช้อยู่ในระดับมาก
สรุปว่าแนวปฏิบัติการพยาบาลมีความเป็นไปได้ในการนำไปใช้ การใช้ในระยะเวลาสั้นทำให้ไม่พบการเปลี่ยนแปลงของการเกิดท่อช่วยหายใจเลื่อนหลุด ดังนั้นพยาบาลควรมีการนำแนวปฏิบัติการพยาบาลไปใช้ในระยะเวลานานขึ้น
