บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลการดำเนินงานกระบวนการประสานรายการยา (Medication Reconciliation: MR) จำนวน ลักษณะ และระดับความรุนแรงของความคลาดเคลื่อนทางยา (Medication Errors :MEs) และวิเคราะห์ความสัมพันธ์ระหว่างการทำ MR กับประเภทและระดับ
ความรุนแรงของ MEs ที่พบในหอผู้ป่วยอายุรกรรม การศึกษานี้เป็นการศึกษาย้อนหลังเชิงพรรณนา (Retrospective descriptive study) กลุ่มตัวอย่าง คือ ผู้ป่วยที่มีประวัติยาเดิมที่เข้ารับการรักษาในหอผู้ป่วยอายุรกรรม โรงพยาบาลสุไหงโก-ลก ตั้งแต่ 1 ตุลาคม 2565 - 30 กันยายน 2566 โดยเก็บข้อมูลจากฐานข้อมูลโรงพยาบาล และแบบบันทึกการประสานรายการยา (Medication Reconciliation Form: MR Form) ผลการศึกษาพบว่าผู้ป่วย 2,760 ราย เมื่อพิจารณาเกณฑ์คัดเข้าออก จะมีจำนวนกลุ่มตัวอย่างที่มีประวัติการใช้ยาที่ใช้ก่อนเข้ารับการรักษา 1,360 ราย ส่วนใหญ่มีโรคความดันโลหิตสูงเพียงอย่างเดียว หรือมีโรคความดันโลหิตสูงร่วมด้วย 1,008 ราย เภสัชกรพบ MEs ในขั้นตอนการเข้ารักษาตัวในโรงพยาบาล (Admission) 324 ราย และขั้นตอนก่อนจำหน่ายผู้ป่วยออกจากโรงพยาบาล (Discharge) 134 ราย และพบ MEs มากที่สุด 3 อันดับแรก คือ การสั่งใช้ยาที่ไม่มีในบัญชียาโรงพยาบาล ผู้ป่วยไม่ได้รับยาเดิมที่ควรจะได้รับ (Omission Error) และการเปลี่ยนแปลงขนาดยาและวิธีใช้ยา ตามลำดับ MEs ที่พบมีความรุนแรงอยู่ในระดับ B และ C เท่านั้น เมื่อวิเคราะห์ความสัมพันธ์ระหว่างการทำ MR กับประเภทและระดับความรุนแรงของ MEs พบว่า การทำ MR มีความสัมพันธ์กับจำนวน MEs ที่พบความรุนแรงในระดับต่าง ๆ อย่างมีนัยสำคัญทางสถิติ (p < 0.001) และการทำ MR เป็นปัจจัยที่มีความสัมพันธ์กับประเภทของ MEs กรณีการสั่งใช้ยาที่ผู้ป่วยไม่เคยได้รับมาก่อน หรือสั่งใช้ยาเดิมที่มีการสั่งหยุดใช้ไปแล้ว โดยไม่มีเหตุผลหรือความจำเป็น (Commission error) อย่างมีนัยสำคัญทางสถิติ (P-value = 0.042) ในขณะที่ประเภทของ MEs อื่น ๆ ไม่มีความสัมพันธ์กับการทำ MR
