การพัฒนานวัตกรรมน้ำมันนวดลังกาสุกะผสมสารสกัดเทียนดำต่ออาการท้องผูก ในผู้ป่วยโรคหลอดเลือดสมองระยะกลาง

ความเป็นมาและวัตถุประสงค์ โรงพยาบาลนราธิวาสราชนครินทร์พบสถิติผู้ป่วยโรคหลอดเลือดสมองมีอาการท้องผูกสูงขึ้นอย่างต่อเนื่อง น้ำมันนวดลังกาสุกะซึ่งเป็นภูมิปัญญาท้องถิ่นของจังหวัดนราธิวาส มาพัฒนาผสมสารสกัดเทียนดำเพื่อศึกษาประสิทธิผลในการกระตุ้นระบบขับถ่ายและประเมินความพึงพอใจของผู้ป่วย โดยน้ำมันนวดลังกาสุกะเป็นศาสตร์การบำบัดจากภูมิปัญญาท้องถิ่นในสามจังหวัดชายแดนใต้ มีเอกลักษณ์เฉพาะของจังหวัดนราธิวาส โดยมีวัตถุประสงค์เพื่อ 1) ศึกษาสภาพปัญหาอาการท้องผูกในผู้ป่วยโรคหลอดเลือดสมองระยะกลาง 2) พัฒนาน้ำมันนวดลังกาสุกะผสมสารสกัดเทียนดำ และ 3) ประเมินประสิทธิผลของการนวดท้องด้วยน้ำมันนวดลังกาสุกะผสมสารสกัดเทียนดำต่อระดับความถี่ในการขับถ่ายและระดับความพึงพอใจของผู้ป่วย
วิธีการวิจัย เป็นการวิจัยและพัฒนา (Research and Development) การศึกษามี 3 ขั้นตอน ได้แก่ 1) ศึกษาสภาพปัญหาอาการท้องผูกจากเอกสารร่วมกับการสัมภาษณ์ผู้ป่วยและผู้ดูแล 2) การพัฒนานวัตกรรมโดยปรับสัดส่วนสมุนไพรในน้ำมันนวดลังกาสุกะผสมสารสกัดเทียนดำจากสมุนไพร 14 ชนิด และ 3) ประเมินประสิทธิผลของการนวดท้องร่วมกับการใช้น้ำมันนวดลังกาสุกะผสมสารสกัดเทียนดำที่ปรับสัดส่วนของสมุนไพรในผู้ป่วยโรคหลอดเลือดสมองระยะกลางที่มีอาการท้องผูก จำนวน 27 ราย เป็นการวิจัยกึ่งทดลองแบบหนึ่งกลุ่ม วัดประสิทธิผลของน้ำมันนวดก่อนและหลังการทดลอง เก็บข้อมูลความถี่ในการขับถ่ายรายเดือน เป็นเวลา 3 เดือน และประเมินความพึงพอใจภายหลังสิ้นสุดการทดลอง เครื่องมือที่ใช้ในการเก็บข้อมูล ประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคล แบบบันทึกความถี่ในการขับถ่าย แบบประเมินความพึงพอใจต่อผลิตภัณฑ์ และวิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติ Repeated Measures ANOVA
ผลการวิจัย พบว่า 1) สภาพปัญหาอาการท้องผูกในผู้ป่วยโรคหลอดเลือดสมองระยะกลาง เป็นผลจากรอยโรค และพฤติกรรมการรับประทานอาหารที่ไม่มีกากใย ส่วนน้ำมันนวดลังกาสุกะในรูปแบบเดิมเน้นการใช้น้ำมันเป็นตัวประสานเพื่อลดการเสียดสีของผิวหนัง และมุ่งเน้นสรรพคุณเพื่อคลายกล้ามเนื้อเป็นหลัก 2) ได้น้ำมันนวดผสมสารสกัดเทียนดำที่มีการปรับสัดส่วนของสมุนไพรเพื่อใช้ในการบรรเทาอาการท้องผูก และ 3) ความถี่ในการขับถ่ายของผู้ป่วยที่เพิ่มขึ้นอย่างต่อเนื่อง โดยค่าเฉลี่ยก่อนการทดลองเท่ากับ 2.89 ครั้งต่อสัปดาห์ และเพิ่มขึ้นเป็น 3.93 ครั้งในเดือนที่สาม ซึ่งผลการวิเคราะห์ Repeated Measures ANOVA พบว่ามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (F3,78 = 46.521,
p < .001) ส่วนผลการประเมินความพึงพอใจ พบว่าค่าเฉลี่ยรวมอยู่ในระดับมากที่สุด (x̅ 4.89, S.D. = 0.32)
สรุปผลการศึกษาและข้อเสนอแนะ ผลจากการศึกษานี้แสดงให้เห็นว่า การนวดท้องเพื่อกระตุ้นการทำงานของลำไส้ร่วมกับการใช้น้ำมันนวดลังกาสุกะผสมสารสกัดเทียนดำ อาจเป็นอีกทางเลือกหนึ่งสำหรับกลุ่มคนที่มีอาการท้องผูก และเป็นข้อมูลพื้นฐานในการนำไปพัฒนาต่อยอดน้ำมันนวดที่บรรเทาอาการท้องผูก ควรมีการศึกษาต่อในระยะยาวเพื่อยืนยันความปลอดภัย และยืนยันถึงประสิทธิผลของการใช้งานจริงอย่างรอบด้าน อีกทั้งงานวิจัยนี้สามารถนำไปต่อยอดและพัฒนาเป็นนวัตกรรมสุขภาพในระดับชุมชนและเชิงพาณิชย์ต่อไป
