ข้อเสนอเชิงนโยบายเพื่อประสิทธิภาพกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภาระกิจโรงพยาบาลส่งเสริมสุขภาพตำบลสู่องค์การบริหารส่วนจังหวัด

ความเป็นมาและวัตถุประสงค์ การถ่ายโอนภารกิจโรงพยาบาลส่งเสริมสุขภาพตำบลทำให้เกิดการเปลี่ยนแปลงเชิงโครงสร้างที่ส่งผลกระทบต่อการดำเนินงานกองทุนการดูแลระยะยาวทั้งการบริหารจัดการกองทุนและการจัดบริการสุขภาพแก่ผู้รับบริการในชุมชน โดยมีเป้าหมายเพื่อพัฒนาข้อเสนอเชิงนโยบายเพื่อประสิทธิภาพกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภารกิจโรงพยาบาลส่งเสริมสุขภาพตำบล
วิธีการวิจัย เป็นการวิจัยแบบผสมผสาน (Mixed methods research) ในพื้นที่วิจัย 747 กองทุน ในจังหวัดที่ถ่ายโอนทั้งจังหวัด โดยดำเนินการวิจัย 3 ระยะ คือ ระยะที่ 1 วิเคราะห์สถานการณ์ และผลกระทบการดำเนินงานกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภารกิจ ระยะที่ 2 พัฒนาแบบจำลองเพื่อประสิทธิภาพกองทุนระบบการดูแลระยะยาว และระยะที่ 3 พัฒนาข้อเสนอเชิงนโยบายเพื่อประสิทธิภาพกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภารกิจการดูแลระยะยาวหลังการถ่ายโอนภารกิจ ระยะที่ 2 พัฒนาแบบจำลองเพื่อประสิทธิภาพกองทุนระบบการดูแลระยะยาว และระยะที่ 3 พัฒนาข้อเสนอเชิงนโยบายเพื่อประสิทธิภาพกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภารกิจ
ผลการวิจัย พบว่า 1) ผู้รับบริการกองทุนระบบการดูแลระยะยาวส่วนใหญ่เป็นผู้สูงอายุ มีอายุเฉลี่ย 77.55 ปี อาศัยอยู่คนเดียวเพียงลำพัง ร้อยละ 1.6 ระยะเวลาการเกิดภาวะพึ่งพิง เฉลี่ย 5.39 และส่วนใหญ่ป่วยด้วยโรคความดันโลหิตสูง ร้อยละ 50.4 รองโรคเบาหวาน ร้อยละ 26.8 2) การจัดบริการสุขภาพภายใต้กองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภาระกิจพบว่า ผู้จัดการดูแลส่วนใหญ่เป็นพยาบาลวิชาชีพ มีบทบาทให้บริการในสถานบริการสุขภาพและจัดบริการดูแลระยะยาวในชุมชนทำให้มีภาระหนัก และพบว่าการจัดบริการดูแลระยะยาวเกือบทุกแห่งไม่มีทีมสหสาขาวิชาชีพเข้าร่วมให้บริการ 3) ประสิทธิภาพกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภารกิจพบว่า งบประมาณของกองทุนลดลงอย่างมีนัยสำคัญทางสถิติ (p value < .001) และร้อยละของแผนการดูแลผู้ป่วยเฉพาะรายแล้วเสร็จลดลงอย่างมีนัยสำคัญทางสถิติ (p value < .001)
สรุปและข้อเสนอแนะ พบข้อเสนอเชิงนโยบายสำหรับหน่วยที่เกี่ยวข้องกับกองทุนระบบการดูแลระยะยาวหลังการถ่ายโอนภาระกิจที่สำคัญ ได้แก่ การพัฒนาระบบการกำกับและประเมินผลการดำเนินงาน การพัฒนาศักยภาพผู้จัดการดูแลและผู้ดูแล การพัฒนามาตรฐานการดูแลระยะยาว เพื่อให้ผู้ป่วยให้เข้าถึงบริการตามชุดสิทธิประโยชน์ภายใต้การบูรณาการ
