การพัฒนาเครื่องมือประเมินความเสี่ยงต่อการเกิดโรคมะเร็งลำไส้ใหญ่และไส้ตรง สำหรับประชาชนในพื้นที่อำเภอนามน จังหวัดกาฬสินธุ์

บทคัดย่อ

โรคมะเร็งลำไส้ใหญ่และไส้ตรงเป็นปัญหาสาธารณสุขสำคัญที่มีแนวโน้มเพิ่มขึ้นอย่างต่อเนื่องในประเทศไทย การประเมินความเสี่ยงรายบุคคลอย่างมีประสิทธิภาพมีบทบาทสำคัญในการส่งเสริมการคัดกรองเชิงรุกและเพิ่มการเข้าถึงบริการสุขภาพ งานวิจัยนี้มีวัตถุประสงค์เพื่อพัฒนาและทดสอบคุณภาพของเครื่องมือประเมินความเสี่ยงต่อการเกิดโรคมะเร็งลำไส้ใหญ่และไส้ตรงสำหรับประชาชนทั่วไป ทั้งเพศชายและเพศหญิง อายุ 50–70 ปี ในพื้นที่อำเภอนามน จังหวัดกาฬสินธุ์             การวิจัยเป็นการศึกษาเชิงวิจัยและพัฒนา (Research and Development) ดำเนินการ 3 ระยะ ได้แก่ (1) การทบทวนวรรณกรรมและแนวทางสากลเพื่อกำหนดกรอบแนวคิดและองค์ประกอบของเครื่องมือ (2) การตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 3 ท่าน ได้ค่าดัชนีความสอดคล้องของเนื้อหา (IOC) ภาพรวมเท่ากับ 0.95 และ (3) การทดลองใช้เครื่องมือในประชาชนจังหวัดมหาสารคาม จำนวน 30 คน เพื่อประเมินความชัดเจนและความเชื่อมั่นของเครื่องมือ ผลการศึกษาพบว่า เครื่องมือที่พัฒนาขึ้นประกอบด้วยปัจจัยด้านข้อมูลประชากร พฤติกรรมสุขภาพ ประวัติครอบครัว และอาการเตือนสำคัญ มีค่าความเชื่อมั่นภายใน (Cronbach’s alpha) เท่ากับ 0.87 แสดงถึงความสอดคล้องภายในในระดับดี และมีความสามารถในการจำแนกระดับความเสี่ยง โดยมีค่าความไว (Sensitivity) เท่ากับ 82.50% และค่าความจำเพาะ (Specificity) เท่ากับ 78.40% จากการศึกษาในกลุ่มตัวอย่างประชาชนอายุ 50–70 ปี จำนวน 405 คน สรุปได้ว่า เครื่องมือที่พัฒนาขึ้นมีความตรงและความเชื่อมั่นในระดับดี สามารถใช้เป็นเครื่องมือคัดกรองเบื้องต้นในระดับปฐมภูมิและชุมชน เพื่อสนับสนุนการตัดสินใจเข้ารับการตรวจคัดกรอง ลดการส่งตรวจที่ไม่จำเป็น และเพิ่มประสิทธิภาพการบริหารจัดการทรัพยากรสุขภาพในระดับพื้นที่
