การเปิดกระเพาะหมักแบบถาวร กรณีศึกษาในโคนมเพศผู้อายุ 15 เดือน

บทคัดย่อ

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาวิธีการเปิดกระเพาะหมักแบบถาวรในโคนมเพศผู้อายุ 15 เดือน โดยใช้วิธีการผ่าตัดแบบครั้งเดียว ภายใต้การวางยาชาเฉพาะที่บริเวณผ่าตัด  เริ่มจากการเปิดช่องที่ผิวหนังบริเวณสวาปทางด้านซ้ายของโคให้เป็นวงกลมขนาดเส้นผ่าศูนย์กลาง 6.5 เซนติเมตร เปิดแยกผ่านชั้นเนื้อเยื่อใต้ผิวหนังและชั้นกล้ามเนื้อ ตัดเยื่อบุช่องท้องให้เป็นช่องวงกลมพร้อมกับเย็บให้ติดกับเนื้อเยื่อกล้ามเนื้อด้วยวัสดุเย็บชนิดไหมละลาย (CATGUT) ตามด้วยเปิดตัดกระเพาะหมักให้เป็นช่องวงกลมพร้อมกับเย็บติดกับผิวหนัง ด้วยวัสดุเย็บชนิดไม่ละลาย (SUPRAMID) และในขั้นตอนสุดท้ายให้ติดตั้งท่อเก็บตัวอย่างเข้ากับช่องเปิดของกระเพาะหมัก การดูแลสุขภาพโคหลังผ่าตัดได้พิจารณาให้ยาปฎิชีวนะทั้งแบบเฉพาะที่และแบบทางระบบ ไม่พบภาวะแทรกซ้อนรุนแรงเกิดขึ้น แต่พบการบวมพองลมใต้ผิวหนังบริเวณสวาปด้านซ้าย ซึ่งบาดแผลได้หายเป็นปกติภายในสี่สัปดาห์หลังการผ่าตัด

Abstract

The objective of this study was to study the permanent rumen fistulation in 15-month-old dairy bull using a one-stage operating procedure under local anesthesia. The left flank skin was marked and dissected as a circle with diameter of 6.5 centimeters, and then blunted dissection through the subcutaneous and muscular tissue. Circular peritoneum was dissected and sutured to muscular tissue by absorbable surgical sutures (CATGUT).   The rumen wall was also marked and dissected as a circle, and then sutured to skin by non-absorbable surgical sutures (SUPRAMID). Finally, the cannula was fitted into rumen aperture.  For post-operative care, both locally and systemically antibiotic treatments were used. No complication appeared, but subcutaneous emphysema was observed on the left flank.  Wound healing process was completed within 4 weeks.
