การศึกษาภูมิปัญญาการแพทย์พื้นบ้านด้านการดูแลมารดาหลังคลอดของโต๊ะบีแด ในอําเภอจะนะ จังหวัดสงขลา

บทคัดย่อ

การดูแลมารดาหลังคลอดตามแนวทางการแพทย์พื้นบ้านมีบทบาทสำคัญต่อสุขภาพสตรีและทารกในชนบทของประเทศไทย โดยเฉพาะในภาคใต้ซึ่งมีความเด่นชัดในวัฒนธรรมมุสลิม การศึกษานี้เป็นการวิจัยเชิงคุณภาพ มีวัตถุประสงค์เพื่อศึกษาภูมิหลังและกระบวนการดูแลมารดาหลังคลอดของโต๊ะบิแด ในอำเภอจะนะ จังหวัดสงขลา โดยคัดเลือกแบบเฉพาะเจาะจง จำนวน 5 ท่าน ใช้การสังเกตแบบไม่มีส่วนร่วม และสัมภาษณ์แบบเจาะลึก วิเคราะห์ข้อมูลโดยการจำแนกเป็นหมวดหมู่และการพรรณนา ผลการศึกษาพบว่า โต๊ะบีแดเป็นเพศหญิง มีอายุ 60 ปีขึ้นไป สืบทอดความรู้มาจากการนิมิตรเห็นครูหมอ กระบวนการดูแลหลักประกอบด้วย หัตถการ ยาสมุนไพร และการให้คำแนะนำในการปฏิบัติตน หัตถการที่เป็นเอกลักษณ์สำคัญ คือ (1) การกดสะกิดหรือดึงบริเวณกล้ามเนื้อหน้าอกเพื่อกระตุ้นน้ำนม (2) การฝังกุนเพื่อให้มดลูกเข้าอู่ และ (3) การทับก้อนเส้าหรือหินร้อนเพื่อขับน้ำคาวปลา สามารถรวบรวมยาสมุนไพรได้
13 ตำรับ ประกอบด้วยตัวยาสมุนไพร 33 ชนิด โดยกล้วยน้ำมีการใช้ซ้ำมากที่สุด (4 ตำรับ) และสมุนไพรหลายชนิดพบการใช้สำหรับมารดาหลังคลอดเป็นครั้งแรก การศึกษาครั้งนี้เป็นการรวบรวมและจัดระบบองค์ความรู้การดูแลมารดาหลังคลอดของโต๊ะบีแดที่ครอบคลุมทุกขั้นตอนในการดูแลสุขภาพ ซึ่งยังไม่เคยมีการศึกษามาก่อน ผลการศึกษาชี้ให้เห็นศักยภาพของหัตถการและตำรับยาสมุนไพรบางรายการในการนำไปพัฒนาเป็นแนวทางบริการการแพทย์แผนไทยสำหรับมารดาหลังคลอด และเป็นฐานข้อมูลสำหรับการวิจัยเชิงคลินิกและการพัฒนาระบบบริการสุขภาพต่อไป
