เวลาเริ่มเกิดและวิถีการดำเนินโรคของภาวะแทรกซ้อนหลอดเลือดขนาดเล็กในเด็ก และวัยรุ่นที่เป็นโรคเบาหวานชนิดที่ 2: การทบทวนวรรณกรรมแบบกำหนดขอบเขต

โรคเบาหวานชนิดที่ 2 ที่เริ่มป่วยในวัยเด็กและวัยรุ่นเป็นภาวะที่มีแนวโน้มเพิ่มขึ้นทั่วโลก และมีลักษณะการดำเนินโรคที่รุนแรงกว่าการเริ่มป่วยในวัยผู้ใหญ่ การทบทวนวรรณกรรมแบบกำหนดขอบเขตนี้มีวัตถุประสงค์เพื่อทำแผนที่หลักฐานเกี่ยวกับเวลาเริ่มเกิดและวิถีการดำเนินโรคของภาวะแทรกซ้อนหลอดเลือดขนาดเล็กในผู้ป่วยเบาหวานชนิดที่ 2 ที่เริ่มเป็นในเด็กและวัยรุ่นก่อนอายุ 20 ปี การศึกษาใช้แนวทางของ Joanna Briggs Institute และรายงานผลตาม PRISMA-ScR โดยสืบค้นจากฐานข้อมูล PubMed Scopus และ CINAHL ในช่วงปี ค.ศ. 2016–2026 จากบทความทั้งสิ้น 107 รายการ คัดเลือกได้งานวิจัยที่เข้าเกณฑ์จำนวน 6 เรื่อง จาก 83 รายการหลังจำกัดช่วงปีพิมพ์ โดยส่วนใหญ่เป็นการศึกษาติดตามระยะยาวจากโครงการ TODAY TODAY2 และ SEARCH ผลการทบทวนวรรณกรรมพบว่า ภาวะแทรกซ้อนของหลอดเลือดขนาดเล็กสามารถเกิดขึ้นได้ตั้งแต่ระยะเริ่มแรกของการวินิจฉัยหรือภายใน 3–5 ปีแรกหลังการดำเนินโรค โดยโรคไตจากเบาหวาน โรคจอประสาทตาเสื่อมจากเบาหวาน และโรคเส้นประสาทส่วนปลายจากเบาหวาน มีอุบัติการณ์สะสมร้อยละ 54.8, 51.0 และ 32.4 ตามลำดับ ภายใน 13.3 ปีหลังการวินิจฉัย ภาวะแทรกซ้อนมีอัตราการดำเนินโรคที่รวดเร็วในช่วงวัยรุ่นถึงวัยผู้ใหญ่ตอนต้น จากการวิเคราะห์วิถีการดำเนินโรค พบว่ากลุ่มที่มีภาวะการกรองของไตสูงผิดปกติในระยะแรก มีความเสี่ยงต่อการเกิดภาวะโปรตีนรั่วในปัสสาวะสูงที่สุด การศึกษานี้ช่วยเติมเต็มช่องว่างทางความรู้เกี่ยวกับระยะเวลาการเกิดภาวะแทรกซ้อน และวิถีการดำเนินโรคในผู้ป่วยเบาหวานชนิดที่ 2 ที่เริ่มเป็นในวัยเด็กและวัยรุ่น นอกจากนี้ ผลการศึกษายังสนับสนุนบทบาทสำคัญของพยาบาลในการคัดกรองและการติดตามอาการเชิงรุกตั้งแต่ระยะแรกของโรค รวมถึงสนับสนุนการพัฒนาแนวทางการคัดกรองภาวะแทรกซ้อนอย่างเป็นระบบตั้งแต่ระยะต้นในผู้ป่วยเด็กและวัยรุ่น ทั้งในระดับหน่วยบริการปฐมภูมิและระดับตติยภูมิ เพื่อชะลอการก้าวหน้าของภาวะแทรกซ้อนได้อย่างมีประสิทธิภาพ
