บันทึกทางการพยาบาลผู้ป่วยออร์โธปิดิกส์ที่ได้รับยาต้านการอักเสบที่ไม่ใช่สเตียรอยด์โรงพยาบาลสงขลานครินทร์: การศึกษาย้อนหลัง

การวิจัยนี้มีวัตถุประสงค์เพื่อ ศึกษาการบันทึกทางการพยาบาลผู้ป่วยออร์โธปิดิกส์ที่ได้รับ NSAIDs ใช้แนวทางการศึกษาบันทึกทางการพยาบาลตามกรอบแนวคิดของกระบวนการพยาบาล กลุ่มตัวอย่างคือ ผู้ป่วยที่ได้รับ NSAIDs พักรักษาตัวที่หอผู้ป่วยศัลยกรรมกระดูกและข้อชาย - หญิง โรงพยาบาลสงขลานครินทร์ ระหว่างวันที่ 1 มกราคม - 30 มิถุนายน พ.ศ. 2560 จำนวน 276 ราย เก็บข้อมูลจากเวชระเบียนโดยใช้เครื่องมือ 2 ส่วน คือ 1) แบบบันทึกข้อมูลทั่วไป (ข้อมูลส่วนบุคคล การเจ็บป่วย การรักษา อาการขณะอยู่โรงพยาบาล การได้รับ NSAIDs และวันจำหน่าย) และ 2) แบบตรวจสอบข้อมูลการบันทึกการพยาบาล (แบบประเมินภาวะสุขภาพแรกรับ บันทึกการให้ยา ปัญหาและความต้องการการพยาบาล บันทึกการพยาบาล บันทึกการวางแผนการดูแลต่อเนื่องและสรุปการจำหน่าย และแบบบันทึกสัญญาณชีพ) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณาได้แก่ การแจกแจงความถี่ ค่าร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน
ผลการศึกษาพบว่า แบบบันทึกที่มีการบันทึกมากที่สุด 1) แบบประเมินภาวะสุขภาพแรกรับ บันทึกครอบคลุมประวัติการดื่มสุรา/สูบบุหรี่/สิ่งเสพติดอื่น ๆ ร้อยละ 98.2 รองลงมา 2) แบบบันทึกการให้ยา มีการ์ดยาอิเลคโทรนิกส์ชัดเจน ร้อยละ 96 3) แบบบันทึกการวางแผนการดูแลอย่างต่อเนื่องและแบบสรุปการจำหน่าย บันทึกชัดเจน ครอบคลุมวิธีการใช้ ผลข้างเคียง การสังเกต/การปฏิบัติตัวเพื่อเฝ้าระวังอาการไม่พึงประสงค์จากการใช้ NSAIDs แหล่งให้ความช่วยเหลือร้อยละ 45.2 4) แบบบันทึกสัญญาณชีพ บันทึกน้ำเข้า-ออก ร้อยละ43.8 5) แบบบันทึกปัญหา/ ความต้องการ เป้าหมายการดูแลบันทึกชัดเจน สอดคล้องกับปัญหา เป้าหมายร้อยละ 0.4 และ 6) แบบบันทึกการพยาบาลไม่มีการบันทึกปัญหา กิจกรรมการพยาบาลเกี่ยวกับ NSAIDs ร้อยละ 100
