การพัฒนารูปแบบการดูแลผู้ป่วยโรคติดเชื้อไวรัสโคโรนา 2019 โดยใช้ Hospitel ในบริบทชายแดนไทย-มาเลเซีย ภายใต้การบริหารจัดการของแผนกผู้ป่วยในโรงพยาบาลสะเดา จ.สงขลา

การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อศึกษาสภาพการณ์ผู้ป่วยโรคติดเชื้อไวรัสโตโรนา 2019 สร้างและพัฒนารูปแบบการดูแลเพื่อป้องกันการแพร่ระบาดของโรค ประเมินประสิทธิผล และปัจจัยความสำเร็จของการนำรูปแบบการดูแลผู้ป่วยโดยใช้ Hospitel ในบริบทชายแดนไทย-มาเลเซีย ดำเนินการ 4 ขั้นตอนคือ 1) ศึกษาสภาพการณ์การดูแลผู้ป่วยโรคติดเชื้อไวรัสโคโรนา 2019 ระลอกแรก 2) พัฒนารูปแบบการดูแลผู้ป่วยจากผลการศึกษาสภาพการณ์และการทบทวนวรรณกรรม 3) ศึกษาประสิทธิผลการนำรูปแบบไปใช้ และ 4) ศึกษาปัจจัยความสำเร็จ เก็บข้อมูลจากบุคลากรสุขภาพ พนักงานโรงแรมและบุคคลที่เกี่ยวข้อง เครื่องมือที่ใช้เป็นรูปแบบที่สร้างขึ้น แนวคำถามการสนทนากลุ่ม และแบบสอบถามการรับรู้เรื่องโรค พฤติกรรมการควบคุมโรค ปัจจัยความสำเร็จ ซึ่งผ่านการตรวจสอบความตรงจากผู้ทรงคุณวุฒิ 3 ท่าน และทดสอบความเที่ยงของแบบสอบถามได้ค่าสัมประสิทธิ์แอลฟ่าของครอนบาค เท่ากับ .80 .83 และ .89 ตามลำดับ วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณนาและ One sample t-test วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์ข้อมูลเชิงเนื้อหา ผลการวิจัยพบว่าหลังการระบาดโรคติดชื้อไวรัสโคโรนา 2019 ระลอกแรกพบผู้ป่วยและกลุ่มเสี่ยงมีแนวโน้มเพิ่มขึ้น ส่วนใหญ่อยู่ในช่วงอายุ 20-29 ปี พบมากใน cluster มัรกัส (สถานที่ประกอบพิธีกรรมทางศาสนาอิสลาม) ชุมชนเขตชายแดนไทย-มาเลเซีย และโรงงานผลิตถุงมือ การทำงานมีการเปลี่ยนแปลงรวดเร็ว มีการประสานงานและใช้ข้อมูลร่วมกัน พบปัญหาอุปกรณ์ไม่เพียงพอ และการจัดการขยะติดเชื้อ
รูปแบบการดูแลผู้ป่วย โดยใช้ Hospitel ในบริบทชายแดนไทย-มาเลเซีย คือ HOME ประกอบด้วย H: Hold hands (ความร่วมมือ) Hotel (ที่พัก) O: Occupancy (การเข้าพักอาศัย) M: Manager & Manpower (ผู้จัดการระบบและกำลังคน) E:Electronic VPN (เครื่องมือที่ใช้เชื่อมโยงข้อมูลระหว่างโรงพยาบาลและโรงแรม) Environment (การจัดการขยะติดเชื้อ) หลังการใช้รูปแบบฯ พฤติกรรมการควบคุมและป้องกันโรคติดเชื้อไวรัสโคโรนา 2019 และการรับรู้โรคติดเชื้อไวรัสโคโรนา 2019 มีคะแนนเฉลี่ยมากกว่า 3.50 อย่างมีนัยสำคัญทางสถิติที่ระดับ .001 ปัจจัยความสำเร็จได้แก่ ความพร้อมของบุคลากรสุขภาพ การสนับสนุนของผู้บริหาร การแบ่งปันข้อมูลและบุคลากร การให้ความรู้เกี่ยวกับโควิดเพื่อสร้างความตื่นตัว การทำงานเป็นทีม ความร่วมมือทั้งภาครัฐและเอกชน การเรียนรู้สิ่งใหม่ การปรับมาตรฐานโรงแรม ตลอดจนการคำนึงถึงความเป็นมนุษย์
