บทคัดย่อ

ความเป็นมาและวัตถุประสงค์ ประเทศไทยมีผู้ต้องขังจำนวนมาก ส่งผลทำให้เรือนจำมีสภาพแออัดและปัญหาด้านสุขภาพ จึงมีการจัดตั้ง อาสาสมัครสาธารณสุขเรือนจำ (อสรจ.) เพื่อช่วยเหลือด้านบริการสุขภาพภายในเรือนจำ การวิจัยนี้จึงมีวัตถุประสงค์เพื่อศึกษาสมรรถนะการปฏิบัติงานบริการสุขภาพและปัจจัยที่มีความสัมพันธ์กับสมรรถนะการปฏิบัติงานบริการสุขภาพของ อสรจ. จังหวัดสกลนคร กลุ่มตัวอย่างคัดเลือกแบบเฉพาะเจาะจง คือ อสรจ. จังหวัดสกลนคร จำนวน 208 คน
วิธีการวิจัย การศึกษานี้วิเคราะห์แบบภาคตัดขวาง (Cross-sectional analytical study) เก็บข้อมูลโดยใช้แบบสอบถามประเมินตนเองเกี่ยวกับปัจจัยที่มีความสัมพันธ์กับสมรรถนะการปฏิบัติงานบริการสุขภาพของ อสรจ. ที่ผู้เชี่ยวชาญ 3 ท่าน ตรวจสอบคุณภาพเครื่องมือ มีค่าความเที่ยงตรงเชิงเนื้อหาอยู่ในช่วง 0.67 – 1.00 และค่าความเชื่อมั่นสัมประสิทธิ์อัลฟ่าของครอนบาคของแบบประเมิน
การสนับสนุนทางสังคมเท่ากับ 0.88 และแบบประเมินสมรรถนะการปฏิบัติงานบริการสุขภาพ เท่ากับ 0.92 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ได้แก่ ค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่ามัธยฐาน และวิเคราะห์ปัจจัยที่มีความสัมพันธ์โดยใช้สถิติไคสแควร์และสัมประสิทธิ์สหสัมพันธ์สเปียร์แมน
ผลการวิจัย พบว่า สมรรถนะการปฏิบัติงานบริการสุขภาพส่วนใหญ่ (ร้อยละ 61.1) อยู่ในระดับสูง (x̅ = 3.73, S.D. = 0.48) ปัจจัยที่มีความสัมพันธ์กับสมรรถนะการปฏิบัติงานบริการสุขภาพของ อสรจ. อย่างมีนัยสำคัญทางสถิติ ได้แก่ อาชีพก่อนต้องโทษ (x̅ = 7.101, p-value = 0.029) การได้มาของอาสาสมัคร (x̅ = 9.238, p-value = 0.002) กองงานที่สังกัด (x̅ = 17.585, p-value < 0.001) และการได้รับอบรมเพิ่มเติม (x̅ = 5.371, p-value = 0.020) และการสนับสนุนทางสังคม มีความสัมพันธ์ในระดับปานกลาง (rs = 0.433, p-value < 0.001) ระยะเวลาการเป็นอาสาสมัคร มีความสัมพันธ์
ในระดับต่ำ (rs = 0.172, p-value = 0.013)
สรุปและข้อเสนอแนะ ควรมีการพัฒนาเครื่องมือประเมินสมรรถนะการปฏิบัติงานบริการสุขภาพของ อสรจ. โดยมีบุคคลากรหลายฝ่ายร่วมประเมิน เช่น เจ้าหน้าที่ผู้คุม พยาบาลเรือนจำ เป็นต้น และควรมีการศึกษาปัจจัยที่มีความสัมพันธ์กับสมรรถนะการปฏิบัติงานบริการสุขภาพของ อสรจ. โดยใช้ระเบียบวิจัยเชิงคุณภาพเพื่อให้ได้ข้อมูลเชิงลึกรวมถึงปัจจัยอื่น ๆ ที่นอกเหนือจากการวิจัยครั้งนี้
