ไฟโลกรุ๊ปและรูปแบบความไวต่อยาต้านจุลชีพของ Escherichia coli ที่คัดแยกจากปัสสาวะของผู้ป่วยหญิงที่ติดเชื้อทางเดินปัสสาวะในโรงพยาบาลระดับตติยภูมิ เขตกรุงเทพมหานคร

บทคัดย่อ
การติดเชื้อระบบทางเดินปัสสาวะจากเชื้อแบคทีเรีย Escherichia coli เป็นปัญหาสุขภาพที่พบได้ทั่วโลก โดยเฉพาะในเพศหญิง งานวิจัยนี้จึงมีวัตถุประสงค์เพื่อศึกษาและวิเคราะห์ความสัมพันธ์ระหว่างไฟโลกรุ๊ป (phylogroup) และความไวต่อยาต้านจุลชีพของ E. coli ที่แยกได้จากผู้ป่วยหญิงติดเชื้อระบบทางเดินปัสสาวะ ผู้วิจัยทำการศึกษา E. coli จำนวน 174 ไอโซเลทที่แยกจากตัวอย่างปัสสาวะของผู้ป่วยหญิงที่ติดเชื้อทางเดินปัสสาวะไม่ซ้ำรายที่เข้ารับการรักษาในโรงพยาบาลระดับตติยภูมิในกรุงเทพมหานคร ระหว่างเดือนพฤศจิกายน 2566 - มกราคม 2567 โดยทดสอบความไวต่อยาต้านจุลชีพด้วยวิธี disk diffusion และจำแนก phylogroup ด้วย quadruplex PCR ผลการทดสอบความไวต่อยาต้านจุลชีพ พบว่า E. coli มีอัตราการดื้อต่อ ampicillin ciprofloxacin และ trimethoprim-sulfamethoxazole อยู่ที่ 88.5 63.8 และ 51.1 % ตามลำดับ นอกจากนี้ 36.8 % เป็นเชื้อที่สามารถสร้างเอนไซม์ Extended spectrum beta-lactamase (ESBLs) และ 61.3 % เป็นเชื้อดื้อยาต้านจุลชีพหลายขนาน สำหรับ phylogroup ที่พบมากที่สุดคือ B2 (61.5%) รองลงมาคือ D (9.2%) C (7.5%) B1 (7.5%) A (5.2%) F (4.0%) E (3.4%) และ unknown (1.7%) โดย phylogroup C มีอัตราการดื้อต่อยาต้านจุลชีพ เช่น ceftazidime, ciprofloxacin และ gentamicin สูงอย่างมีนัยสำคัญ (p<0.05) และเป็นกลุ่มเดียวที่พบการดื้อต่อ carbapenem ในขณะที่ phylogroup E มีอัตราการดื้อต่อยาหลายขนาน (MDR) สูงอย่างมีนัยสำคัญ การศึกษานี้แสดงให้เห็นว่า E. coli จากผู้ป่วยหญิงที่ติดเชื้อทางเดินปัสสาวะมีอัตราการดื้อต่อยาสูง สะท้อนถึงความจำเป็นในการเฝ้าระวังและควบคุมการแพร่กระจายของเชื้อดื้อยาดังกล่าว
